I First Name* Last Name* and First Name Last Name for good and valuable consideration, the receipt of which is hereby acknowledged, hereby irrevocably authorize Carla Johnson Photography to use photographs of me and or my property and authorize her and her assignees, licensees, legal representatives and transferees to use and publish (with or without my name, company name, or with a fictitious name) photographs, pictures, portraits or images herein described in any and all forms and media and in all manners including composite images or distorted representations, and the purposes of publicity, illustration, commercial art, advertising, publishing (including publishing in electronic form on CDs or internet websites?, for any product or services, or other lawful uses as may be determined by the Photographer/Carla Johnson Photography. I further waive and and all rights to review or approve any uses of my likeness, any written copy of finished product. I acknowledge and agree that publication of said photos confers no rights of ownership or royalties whatsoever. I hereby release Carla Johnson Photography and any third party in connection with my participation and/or the minor children listed below.

I am full legal age and/or am a parent/legal guardian to all children under the age of 18 listed and have read and fully understand the terms of this release.

IMPORTANT NOTICE REGARDING COPYRIGHT: It is illegal to copy or reproduce the Clients photographs elsewhere without Carla Johnson Photography permission, and violators of this Federal Law will be subject to its civil and criminal penalties. All photographs, both on Carla Johnson Photography website, blog or Facebook page, including all potential proofs and purchased prints, are copyrighted, reproduction of any image by any means, including copying, re-printing, screen captures, and other means of print reproduction are strictly prohibited and are a violation of copyright law. Additionally, it is illegal to post any of Carla Johnson Photography work on the Internet, blogs, and social networking sites, without the express permission of the photographer.

I specifically request that Carla Johnson Photography withhold from sharing our photos on her blog, in advertising promotions, or on Social Media, in order to protect our privacy. (This does not include posting photos to an online gallery for the client’s personal use and ordering.)
