TUITION AND FEES: By signing this document, I understand that tuition is due by the first class of each month, and that a $15 late fee will be charged if paid after the 15th of each month. Tuition is charged by the month, no matter how many classes my child attends. I understand that there is a registration fee, and a recital fee per family. I acknowledge that if my child withdraws from classes during the year, I must notify the Dance Factory staff thirty days prior to the date of withdraw, otherwise payment will be expected as usual. I understand that if my account is not paid in full within 60 days, my child will not be allowed to participate in class until further arrangements have been made.
PHOTO & VIDEO RELEASE: The Dance Factory of New Smyrna Beach, Inc. is including photos & video of students, teachers, and studio activities on its website and printed materials. I understand that these pictures will be accessible to anyone with internet access. I give my permission for the Dance Factory of New Smyrna Beach, Inc. to use photos of my child/children on its website, on Facebook, in printed materials, and in electronic forms of communication. I understand that if I do not want images of my child to be used, I will indicate this in writing and the signed letter will be attached to this document.
DISMISSAL FROM CLASS: The Dance Factory of New Smyrna Beach, Inc. provides a fun, rewarding, and challenging experience in dance, to students of all ages and abilities. This results from the collaboration among parents, students, and teachers. Class participation, a positive approach to learning, adherence to policies, and parental support are important factors influencing the experience of your student and others enrolled in class. In order to maintain a welcoming atmosphere, the Dance Factory of New Smyrna beach, Inc. reserves the right to dismiss any student, as it sees fit.
CONDITIONS OF PARTICIPATION: You hereby acknowledge and agree that your student’s use of the Dance Factory of New Smyrna Beach, Inc. (herein known as the “studio”) facilities, services, equipment or premises, involves risk of injury to persons and property, including those described below, and students assumes full responsibility for such risk. Inconsideration of being allowed to enter the studio and/or to participate in any related studio events, for any purpose, whether or not the studio, including but not limited to, instruction, observation, use of facilities, services or equipment, or participation in any way. Student agrees to the following: Student and/or family members hereby release the Dance Factory of New Smyrna Beach, Inc., its directors, owners, employees, agents, all studio personal, etc. and the Volusia County Schools from liability to student and student’s personal representatives, assigns, heirs and next of kin for any loss and/or damages, and waives any claims or demands therefore, on account of injury to student’s person or property, including injury leading to the death of student, whether caused by the active or passive negligence or otherwise of studio parties or anyone else, while the student is in, upon, or about the studio premises or participating in any way in any studio event whether or not held at the studio. Students also agree to protect all studio parties from any loss, liability, damage or cost studio parties may incur due to the presents of student in, upon or about studio premises or observation and/or participation in any studio activity whether caused by the negligence or otherwise of studio parties or anyone else. You represent (A) a student who is in good physical condition, has no disabilities, illness, or other condition that could prevent the student from dancing and/or exercising without injury or impairment of health. (B) You have consulted a physician concerning a dance or exercise program that will not risk injury to student or impairment of student’s health. Such risk of injury includes, but is not limited to: injuries arising from dance, tumbling and/or exercise at the studio or ANY studio related event. Students further expressly agrees that the foregoing release, waiver and agreement is intended to be as broad and as inclusive as permitted by the laws of the State of Florida and that any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding continue in full force and effect. Student, Parent and/or Guardian has read this release and waiver of liability and indemnity clause and agrees that no oral representations, statements, or inducements apart from this foregoing agreement have been made. In signing below, you consent too and understand all of the policies of the Dance Factory of New Smyrna Beach, Inc. Your give your consent for your student, yourself or any family member to take part or participate. *
