    This is a pre-consult form to ensure you are a proper candidate for this treatment. ALL appointments require a deposit/booking fee to be confirmed! 

    Attaching a photo of what your brows look like naturally and communication of what you’re looking for, helps set realistic expectations as not everyone is a candidate for this service.

  • *****Microblading appointments have a 48 hour cancellation policy. No show appointments require full fee paid. You are not required to pay the booking fee at this time but once a date and time is agreed upon, a $100 booking fee is required to hold the appointment. It will be taken off the total on the day of your appt. The booking fee is non-refundable. By completeing this form you understand that not being truthful or disclosing information on the intake or request forms may disqualify you from being a candidate for microblading and a fee implemented at the time of the appointment.
