Student Policies Agreement
- An annual $40 Registration Fee is due with this form (payable online). The Registration Fee for additional immediate family members is $20 per student. A separate Registration Form is required for each student.
- Upon receipt of this form, each will be contacted for teacher placement. After placement, all lessons are scheduled through the teacher.
- Upon receipt of this form and the $40 registration fee, each student will be issued a keycard. This card must be used for building access via the Office Entrance. All students are to check-in on the SFA kiosk.
- Lesson Fees begin at $28 per 1/2 hour lesson. Fees for hour-long lessons are set by the teacher. Lesson fees are due to the teacher in advance for each two month period and are non-refundable. Make up lessons are at the discretion of the teacher.
- Each student is required to contact their teacher at least 24 hours in advance of a missed lesson.
- Termination of lessons must be made with 30 days notice to the teacher.
- All concerns/issues should be brought in writing to the SFA Executive Director.
