Long Service Award Application Form

Employees are entitled to a £125 gift voucher upon reaching 25 Year's Service with Nottingham City Council. Line Managers must complete this form for employees reaching their Long Service date in order for the award to be processed.
Gift Vouchers will be sent to the employee in the form of an e-cheque via email. Please ensure you provide the correct email address for the employee, this can be a personal email address if they prefer.

If the employee does not have access to email, please contact the Works Perks Team on 0115 876 3999 and a suitable alternative can be arranged.
