Godparent Information Form
  • Godparent Information Form

*one form to be completed by each godparent or Christian witness
  • Thank you for your willingness to serve as a godparent!

    What is a godparent?

A godparent, together with the parent(s), presents a child for baptism. A godparent is ready and able to help the newly baptized on the road of Christian life (Can. 872, CCC 1255).

In the short video below, Father Mike Schmitz shares what it means to be a godparent. He explains, “you don’t have to be perfect to be a godparent! All you have to do is do your best to love Jesus, to love the Church, and to love this child [to be baptized].”

  • Who can be a godparent?

    REQUIREMENTS:

    • age 16 years or older
    • has received the sacraments of Baptism, Eucharist, and Confirmation
    • a practicing Catholic (should regularly attend Sunday Mass and be a faithful Catholic)
    • If the godparent is married, he/she must be married in the Catholic Church
    • Cannot be the parent or the step-parent of the child to be baptized

A child receiving baptism may have one godfather and one godmother. Only one godparent is required, if parents prefer.

Children may instead have one Catholic godparent along with one Christian Witness, a non-Catholic, baptized Christian from another faith tradition

Per the Diocese, unless the godparent is a registered parishioner of St. Maria Goretti, a "letter of goodstanding" is required from the pastor of the godparent's parish before the baptism can be scheduled.
