Service Unit Reimbursement Request

Service Unit Directors may request reimbursement for financial aid provided to key adult volunteers up to $100 annually. You may only submit this form once per membership year.

Please make sure all requests are submitted by July 1 for the current membership year. For example:

2025-2026 deadline for submissions is July 1, 2026.

To request reimbursement, Service Unit Directors must complete the following form. Only service units with last year's finance report on file can request reimbursement.
