  Waiver and Release of Liability
  
  As parent or legal guardian of the student(s) named above, I understand and acknowledge the risks associated with the sport of gymnastics, parkour, dance and other related activities, including paralysis and other catastrophic injury, as well as medical expenses and damages that may result or be associated with the sport of gymnastics, parkour, dance, and any related activities as referenced above. This Waiver and Release of Liability applies to Kids Gone Wild, LLC, dba Scottsdale Gymnastics & Trampoline, its board of directors, officers, employees, instructors, agents, representatives, any independent contractors, including any and all instructors, landlords, and its successors and assigns, at all of its business locations where activities are conducted by it including but not limited to the following locations in Scottsdale, Arizona, 9321 N. 94th St., 9419 E. San Salvador 101A/101B, 9322 N 94th Way 101 and 9445 E. Doubletree Ranch Rd. This waiver and release of liability shall not cover those situations where loss, injury or damage to the above participant is the result of the intentional and/or reckless conduct by any of the parties included in this waiver and release. The undersigned further agrees to indemnify and save and hold Scottsdale Gymnastics & Trampoline harmless, including all other parties identified above from any liability arising out of the negligent or intentional conduct of participants, parents, family members or parties invited upon the premises by the participant which results in loss, injury or damage to any other party.
  
  Release Authorization
  
  Without compensation to me or the student(s), I hereby grant to Kids Gone Wild LLC dba Scottsdale Gymnastics & Trampoline the absolute right and permission to copyright, publish, and use photographic portraits, pictures, or videos of the myself for use through reasonable promotion of gymnastics and sport conducted by Scottsdale Gymnastics & Trampoline. I hereby waive any right that I may have to inspect or approve the finished material as long as the matter is within reason and is not deemed to be socially inappropriate for use.
  
  Medical Release
  
  I hereby authorize and give my consent to Kids Gone Wild LLC dba Scottsdale Gymnastics & Trampoline including any of its instructors or other authorized employees to provide emergency medical care and to give authority to any emergency unit, hospital or doctor to render immediate aid that might be required for the treatment of the above named participant(s) in the event of any emergency either on the premises of Scottsdale Gymnastics & Trampoline or during the course of any sports event involving the participant as a Scottsdale Gymnastics & Trampoline member and/or non-member.
  
  Verification and Release
  
  As Legal parent, guardian or responsible party of this student, I hereby verify by my signature below that I accept the conditions of the waiver, release and indemnification; and I knowingly with full understanding of the risk involved, assume the risk of participating in the events provided by Scottsdale Gymnastics & Trampoline.
  
  Acknowledgment
  
  I hereby acknowledge that I have read the entirety of this document and understand that it includes a Waiver and Release of Liability, and that if there is any part of it I did not understand, that prior to signing it, I sought the advice of legal counsel.
- Policies, Procedures & Gym Rules:
  
  TUITION:
  
  •Auto-pay is the only form of payment for tuition
  •Auto-pay is run on the first of each month
  •Auto-pays that are declined must be resolved by the next day, or there will be a $20.00 late fee assessed to the account.
  •Accounts that are not current by the 1st day of class will result in the student being dropped from the class
  •Cancellation of Auto-pay must be done 30 days prior to the drop date via email to funhappens@scottsdalegymnastics.com
  •There are no refunds or pro-ration for tuition. However new students entering the program after the 1st class will have their tuition prorated.
  •Tuition is based on 4 weeks however if there are 5 weeks the account will be adjusted accordingly. Tuition is always adjusted when there is a recognized gym closure.
  •The annual membership fee is $45 for the first child and $20 for each sibling thereafter
  •Sibling discounts off regular tuition are 10% 2nd child, 15% 3rd child, and 20% thereafter
  •Special Events and camps are prepaid there are no refunds or makeups for these
  
  HOLIDAYS: The following holidays are observed and tuition is adjusted accordingly. New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving and Christmas.
  
  TRIAL CLASS: This is a no-obligation class for a new student to assist us in placement. Trial classes will only be charged if the student enrolls. You are welcome to try additional classes, however there will be a one-time class fee.
  
  MAKEUPS: Students must be currently enrolled in class. They will have one month from the missed class to schedule a makeup in a class that is equal in time or they shall forfeit the make-up. Open Gyms are available for make-ups however it must be scheduled through the front desk prior to Open Gym. All scheduled makeups will count in attendance unless cancelled 24 hours in advance. THERE ARE NO REFUNDS, CREDITS OR PRO-RATIONS FOR MISSED CLASSES
  
  LATE POLICY: For health and safety reasons, your child will not be admitted to class if he or she is more than 15 minutes late.
  
  SICKNESS POLICY: Do not bring sick children to the gym. Any child showing signs of sickness will be asked to leave the facility, no exceptions. Please schedule a makeup.
  
  PROPER ATTIRE: All students with long hair must keep it securely pulled back. A leotard or a shirt that is not loose fitting is best and stretch shorts without pockets are optional for girls and sport shorts and t-shirt for boys. Jewelry is not permitted. All parkour students must have tennis shoes to participate.
  
  PROPER BEHAVIOR: Respectful and safe behavior is expected at all times. Unacceptable behavior such as; disrespect for others, pushing, hitting, line cutting, and bad language will not be permitted and the child will receive a warning, 2nd warning will result in loss of turn, 3rd warning will result in being asked to leave class and meeting with the a parent. Students who are continually disruptive will be asked to find other instruction.
  
  SAFETY: Instructors shall go over gym safety during instruction. There are safety posters throughout the gym to assist students in remembering the rules.
  
  LOST AND FOUND: All items that are found in the gym will be placed in our lost and found bin and held for 2 weeks after this period the items are donated to Goodwill. SGT IS NOT RESPONSIBLE FOR LOST OR STOLEN ITEMS
  
  Please remember for liability reasons parent and siblings are not allowed in the gym. Viewing is permitted in designated areas only. Please keep all non-participating siblings with a parent or guardian at all times, it is important to us that you and your child have a safe, fun and rewarding experience at our facility.
