GREGORY'S GIFT OF HOPE REQUIRES THAT ALL OUR ADOPTED DOGS ARE SPAYED/NEUTERED. All dogs over 6 months old will already be altered before adoption BUT all dogs under 6 months of age will not. The expense will be your responsibility. If you have any questions or reservations about spay/neutering, please explain.

Too often pets are surrendered to rescues or become homeless because no arrangements were made in regard to such unfortunate circumstances. ADOPTION TERMS AND AGREEMENT

Please read carefully. By initialing a paragraph, you have agreed to that term. Omission of information or failure to answer all questions will result in this application being declined.
1) I, hereafter referred to as the "adopter", agree that the aforementioned dog is being adopted by me solely, as a pet for myself and/or my immediate family. I agree that I will not sell, give away, or otherwise dispose of said dog to any person, dealer, retailer, auction or other entity, for any reason.*

2) If I determine a need to surrender this dog, I will first contact GGOH and have a trainer evaluation performed in my home to determine if there are options available to work towards keeping the dog vs surrendering it back to us. If training sessions would be beneficial to the dog, I agree to have a GGOH trainer conduct training sessions with me and family at my home to help us reach an agreeable training goal.*

3) I agree and understand that any surrendering back to GGOH will incur a $100 surrender fee and any said dog will need to be up to date on all vaccinations--copies to be provided by current veterinarian clinic in use. Failure to provide and abide by these conditions will result in a $500 fine, enforceable by court order.*

4) I agree to care for this dog in a humane and responsible manner and to provide shelter, food, water, and proper veterinary care. I further agree that this dog shall reside inside my home and WILL NOT BE ALLOWED TO ROAM FREELY OUTSIDE. I agree that any canine adopted from GGOH will NOT be debarked.*

5) I agree to pay the adoption fee to cover the rescue's expenses in caring for this dog prior to adoption. THIS FEE IS NON-REFUNDABLE. $500 Puppies (under 12 months old), Adult Dog (12 months thru 8 years) $325, Seniors (9 years and older) $200. *

6) I understand and agree that Gregory's Gift of Hope Inc makes no representations or warranties (expressed or implied) about the animal's temperament and is absolved from any liabilities for future damage or injuries caused by said dog*

7) I understand that the adoption decision is dependent upon many factors, including but not limited to, the compatibility of the family and home to the individual dog as determined by interview and home visit by a trained representative of GGOH Inc.*

8) We require our adopters to enroll into training classes, board and train, or in your home training (by a qualified trainer) for all dogs under 24 months old, and strongly recommend it for all dogs. We feel strongly that training classes help strengthen bonds, communication, and enhance good manners. We will follow up to verify a training course has been completed and require proof of completion. I agree to complete a positive training course within 6 months from date of adoption. *

TO VISIT/MEET ANY OF THE GREGORY’S GIFT OF HOPE RESCUE ANIMALS YOU MUST READ AND SIGN THIS WAIVER/RELEASE FORM: I and family members agree to wear a face mask, if requested, during a rescue animal visitation/meeting and/or at any GGOH Rescue Animal event. Although precautions are taken, the risk to have contact with individuals who have been exposed to Covid-19 does exist and it is impossible to eliminate the risk that I or family member could be exposed to and/or become infected through contact with or close proximity with an individual with this disease. I hereby fully and forever waive, release, and hold harmless, Gregory’s Gift of Hope Animal Rescue/Shelter, including board members, foster homes, and volunteers, from actions, claims or every nature whatsoever, damages and judgements which I may have now or in the future against Gregory’s Gift of Hope Animal Rescue/Shelter for injuries or sickness to myself and family members, known or unknown, and arising out of myself or family member as a visit, volunteer, and/or prospective adopter of Gregory’s Gift of Hope Animal Rescue/Shelter. I, THE UNDERSIGNED, HAVE READ THIS WAIVER AND RELEASE AND UNDERSTAND ALL OF ITS TERMS. I EXECUTE IT VOLUNTARILY WITH FULL KNOWLEDGE OF ITS SIGNIFICANCE.*
