APPLICATION FORM GUIDE
This Application Form applies to all schools provided by Youth With A Mission Canberra.

We ask you to answer all the questions on the application. If a question does not apply to you, write N/A (not applicable) in the space provided. Husbands and wives enrolling as students in the same school must complete separate application forms though only one person needs to complete information about any accompanying children.

APPLICATIONS: It is recommended that applications from overseas students be completed twelve weeks prior to the start of the school to ensure time for visa processing. Applications will be still be received up to four weeks before the start of school, but for overseas students there is the risk that your visa will not be processed in time for the start of the school.

REFERENCES: We require two personal references for each person applying for a school at Youth With A Mission Canberra. You can choose from the following:
- Pastor or Spiritual leader.
- Employer or business associate.
- Teacher.
REGISTRATION FEE: Your registration fee of AU$50.00 (AU$80.00 per married couple) must be sent with the application or as soon as possible after the submission of the application. This fee is not refundable. Your application cannot be processed without the registration fee (unless there has been a prior arrangement made).

MEDICAL REQUIREMENTS: The confidential medical forms require the applicant's attention and it's important for them to be completed honestly. If necessary an additional form will be sent for your doctor to complete.

Medical forms are also required for all accompanying dependents 16 years of age and older.

PASSPORTS: Everyone attending a YWAM school should have a valid passport with an expiration date of at least 6 months after the conclusion of the school.

VISAS: Please do not apply for a visa until you receive confirmation of enrolment from YWAM Canberra along with the appropriate papers. If you have a current Australian visa, please tell us what type of visa you have and when that visa expires.

STUDENT HANDBOOK: Please read the Student Handbook, as this describes the policies under which courses of YWAM Training are run. YWAM Training is a registered Training Provider #02026G: and CRICOS #0449.

Before we can process your application you must pay your $50.00 ($80.00 for couples) NON REFUNDABLE application fee. There are two ways that you can make this payment:

ACKNOWLEDGEMENT OF FINANCIAL RESPONSIBILITY
I understand that payment of the school tuition fees must be paid before or upon my arrival unless otherwise agreed with leadership before my arrival. I am fully aware of my financial obligations both to God and to the students and staff of the school.

I also understand that should I choose to the leave the school after a certain date and prior to the completion of the program, I am not entitled to a refund. (See the Refund Policy in the Student Handbook.) I promise to keep my word and pay in a timely manner all expenses incurred during my involvement with Youth With a Mission. I also commit to taking all the necessary steps to meet this promise, and accept the help and suggestions of my future leaders.
