  • Our Dream Dog program is named in honor of Skye, the first dog released from our training program. With the help of the Make-A-Wish Foundation, Skye found a special home with a young boy battling kidney cancer.

At ACTS Service Dogs, we believe every dog has a purpose and a story to tell, whether they graduate as a service dog or follow a different path.

We invite you to share your story with us as you complete this application. Tell us about yourself, your home, your lifestyle, and what welcoming a Dream Dog into your life would mean to you.

If you are selected to adopt one of our Dream Dogs, your story will become part of their story. The more we know about you, the better we can find the right match.

Our Dream Dogs are not placed on a first come, first served basis. Each dog is thoughtfully matched with the home and family best suited to their individual personality and needs.

  • By submitting this application, I acknowledge and understand the following:

    1. Application and Placement: Submitting an application does not guarantee that I will be matched with or offered a Dream Dog. There is no guaranteed timeline for placement, and dogs are matched based on their individual needs and suitability for each home, not the order in which applications are received.

    2. Right to Decline: I may decline any dog offered to me without affecting my eligibility or consideration for future Dream Dogs.

    3. Age and Reasons for Release: Dream Dogs are typically released from the ACTS training program between 2 months and 2 years of age. Dogs may be released for various reasons, including medical, behavioral, or other factors that make them unsuitable for service dog work.

    4. Disclosure of Information: ACTS Service Dogs will share all known relevant medical, behavioral, and training information about a dog being considered for adoption to the best of its knowledge and ability.

    5. Responsible Dog Ownership: I understand that adopting a dog is a long-term commitment. I am prepared to provide a safe, loving home and assume responsibility for all ongoing expenses, including food, veterinary care, grooming, training, and other necessary care.

    6. Adoption Fees: I understand that Dream Dog adoption fees currently range starting from $5,000 to $10,000, depending on the individual dog, and are subject to change.
