  • Registration for Assessment for Black Belt Promotion

    This is your formal request to be considered a candidate for Black Belt promotion. If you have passed the required number of Pre Exams and all auxillary requirements on the back of your attendance card, this is your next step toward your Black Belt Exam.

    This is NOT your Black Belt application.

    Submitting this form lets us know of your intention to test, but it includes no guarantee of your promotion. That will be up to you. In order to be offered an application for Black Belt promotion, you will need to prove your ability and demonstrate a black belt attitude throughout a series of assessments.

  • You will be presented with a series of statements to which you must agree and initial. Be sure to read each carefully. Your initials will indicate your complete understanding and agreement with each statement.

  • I have completed my required Color Belt Exam assits.
    (5 for Poom/BCD, 6 for 1st Poom/Dahn, 12 for 2nd Dahn)
  • I have participated in at least one tournament at my current
    belt level.
     
     
     
  • I have completed my Black Belt Exam assist requirement.
     
     
     
  • I will have attended at least 50 classes prior to the day
    of the Assessment.
     
     
  • I have secured partners for color belt self defense, ho shin
    sool/knife self defense, and staff self defense (2nd degree only).
     
     
    -Your color belt self defense partner cannot be a Black Belt
    candidate (someone who is testing for Black Belt).
     
    -Your Ho Shin Sool partner CAN be another candidate.
  • I understand that there must be a minimum of five
    candidates for Lee Brothers to be able to run this Black
    Belt Exam.
     
  • I understand that submission of this registration form does not
    guarantee that I am eligible to test. Only if I am able to
    demonstrate the required skills and attitude will I be offered
    an Application for Black Belt Promotion. I will willingly delay
    testing if my master deems me unprepared.
  • For High School Age and Under Only:
    I am aware that I will need to submit
    my most recent report card when the
    application is due at Part 3 of the Exam.

  • After you submit this form, you are invited to come to the information meeting which will go over your next steps. You do not need to start working on your essay or practice runs quite yet, as you will not become an offical candidate for the exam until you pass the assesmment. Those items, as well as the application and exam fee of $450 will not be due until the final mandatory practice. The dates of the information meeting, assessment, and mandatory practices can all be found on our yearly planner. Please reach out to us if you have any questions.
