I agree to keep a clean social media presence, and conduct myself with honor and character both on and off the field. *

I understand that fighting at practice, games or events will not be tolerated and can result in suspension or release from the team or league. *

I hereby represent and warrant that, to the best of my knowledge and belief, the above named athlete is physically and mentally able to participate in the Alabama Steel Semi Professional Football program involving sports training and competition. I hereby give permission for the above named athlete to participate in the Alabama Steel Semi Professional Football program and adhere to the Code of Conduct.

In consideration for the ALABAMA STEEL SEMI PROFESSIONAL FOOTBALL program providing the opportunity for above named athlete to participate in the 2023-24 PRE-SEASON AND 2024 SEASON, I hereby release The ALABAMA STEEL SEMI PROFESSIONAL FOOTBALL ORGANIZATION and its officers, directors, volunteers, agents, contractors, supporters, or any other person associated with THE ALABAMA STEEL SEMI PROFESSIONAL FOOTBALL ORGANIZATION, FIELD and LEAGUE, from any and all claims, damage, or injury that the above named athlete may suffer as a result of participation in the COMBINE, TRYOUTS, STRENGTH AND CONDITIONING SESSIONS, PRE-SEASON AND 2023-2024 SEASON programs. In addition, I hereby agree to indemnify, defend and hold THE ALABAMA STEEL SEMI PROFESSIONAL FOOTBALL ORGANIZATION, FIELD, OR THE LEAGUE harmless from any and all claims for loss, damage (including attorneys’ fees and costs, including, but not limited to experts and consultants’ fees, loss of wages, etc.), liability, sickness, death, or injury to the person or property arising from or related to the above named athlete’s participation in the ALABAMA STEEL SEMI PROFESSIONAL FOOTBALL ORGANIZATION’S COMBINE, PRE-SEASON, AND/OR 2023-2024 SEASON programs.

I assume all risks and hazards involved in, or incidental to, the participation of above named athlete in the ALABAMA STEEL SEMI PROFESSIONAL FOOTBALL program and hereby consent to above named athlete to receive first-aid and/or emergency care by a qualified Emergency Medical Technician, Nurse, Physician, Chiropractor, Trainer or other medical personell in the event of any injury during any SPORTS OR EVENT for THE ALABAMA STEEL SEMI PROFESSIONAL FOOTBALL ORGAINZATION OR LEAGUE. I agree that I am solely responsible for obtaining, maintaining, both physically and financially for medical, health and life insurance or lack there of, in the event of sickness, virus, injury, accident, or death. I release and hold harmless the ALABAMA STEEL SEMI PROFESSIONAL FOOTBALL ORGANIZATION, LEAGUE, FIELD, VOLUNTEERS, STAFF, OR ANY REPRESENTATVES THEREOF, OF ANY LEGAL AND/OR FINANCIAL RESPONSIBILITY OR LIABILITY IN REFERENCE TO SICKNESS, VIRUS, INJURY, ACCIDENT OR DEATH DUE TO MY PARTICIPATION IN OR TRANSPORTATION TO AND/OR FROM ANY EVENT, PRACTICE, GAME, COMBINE, TRYOUT, VOLUNTEER WORK OR OTHERWISE.

I agree to provide all pertinent medical information to THE ALABAMA STEEL SEMI PROFESSIONAL FOOTBALL ORGANIZATION and to assist The ALABAMA STEEL SEMI PROFESSIONAL FOOTBALL ORGANZIATION so that adequate precautions can be made and so that appropriate care can be provided to above named athlete during THE ALABAMA STEEL SEMI PROFESSIONAL FOOTBALL ORGANIZATION 2023-2024 PRE-SEASON AND/OR SEASON programs. I agree to have all of above named athlete’s necessary medication on hand with proper instructions during any ALABAMA STEEL SEMI PROFESSIONAL FOOTBALL GAME OR EVENT.

I hereby grant THE ALABAMA STEEL SEMI PROFESSIONAL FOOTBALL ORGANIZATION, it's affiliates, franchises, advertising and promotional agencies, and their agents, the irrevocable, unrestricted right to use, publish, display and distribute materials bearing the above named athlete’s name, voice, likeness or any other identifiable representation of above named athlete or family members. These materials may appear in any form, style, color or medium whatsoever (including photographs, videotapes, films, sound recording, software, drawing, prints, broadcast, internet and electronic media). I agree that all material containing any identifiable representation of above named athlete shall be and remain the sole and exclusive property of THE ALABAMA STEEL SEMI PROFESSIONAL FOOTBALL ORGRANIZATION. I hereby release and forever discharge THE ALABAMA STEEL SEMI PROFESSIONAL FOOTBALL ORGANIZATION from any and all liability and damages relating to the use of above named athlete’s name, voice and any other identifiable representation of above named athlete. I have agreed to the above in consideration of the opportunity given to above named athlete by THE ALABAMA STEEL SEMI PROFESSIONAL FOOTBALL ORGANIZATION to appear in these materials.

I acknowledge that I am responsible for paying all player's fees, registration fees and season/league fees to the Alabama Steel Semi Professional Football Organization. I understand even if I decide to not proceed to finish the season at anytime during the pre-season, season or post season, I am not due a refund at any time for any monies paid in. ALL DUES, FEES, AND/OR MONEY COLLECTED IS NON REFUNDABLE.

I acknowledge by entering into this registration with the ALABAMA STEEL SEMI PROFESSIONAL FOOTBALL ORGANIZATION for the 2023-2024 pre-season and season that I am not elgible to participate in any other Semi-Professional Team or Developmental Team games, practices, or activities as a player, coach, volunteer, or staff member unless fullly released without restriction or conditions by the ALABAMA STEEL SEMI PROFESSIONAL FOOTBALL ORGANIZATION within the same league. The ALABAMA STEEL RETAINS THE RIGHT TO RESTRICT CERTAIN ACTIVITIES OF THE ABOVE NAMED PLAYER IN THE BEST INTEREST THE ORGANIZATION FOR THE ENTIRE 2023-2024 PRE-SEASON, REGULAR SEASON AND PLAYOFF SEASON.

By submitting this document, participating in any practice or event hosted by or on behalf of the ALABAMA STEEL SEMI PROFESSIONAL FOOTBALL ORGANIZATION I acknowlege my consent to have fully read, understand and be full agreement with this document.
