1. The undersigned hereby agrees to make a weekly payments to ON THE WAY PRIVATE SCHOOL TRANSPORTATION SERVICES, for transportation services for my child named on this form, with the purpose and convenience of no worries of taking and picking up my child from school

3. This is based on a weekly service, it is not a contract and be canceled at any time within a 15 day notice. If we don't receive the 15 day notice the deposit will be non-refundable. The deposit can be used for the last week of service.

6. I understand if I have an unpaid balance to ON THE WAY PRIVATE SCHOOL TRANSPORTATION SERVICE and do not make satisfactory payment arrangements, my account may be placed with an external collection agency. I will be responsible for reimbursement of the fee of any collection agency, which may be based on a percentage at a maximum of 35% of the debt, and all costs and expenses, including reasonable collection and attorney’s fees incurred during collection efforts.
