    2. An approval email will be sent back to you upon review.
    3. This form must be submitted at least 2 weeks before the event.

  • Room Arrangement

  • If you need tables and chairs arranged in a specific configuration please submit that arrangement to the parish office. Or, you can upload a file below showing the requested configuration.

  • If "yes," a "PAC Kitchen Agreement" will be sent to you in the approval email.

  • Please Note: Because of the sensitivty of this equipment, staff members ONLY are permitted to set up this equipment.

    • Please leave the facility clean and orderly.
    • Your group is responsible for room(s) you use.
    • Extra cleaning supplies (brooms, etc.) are in the storage area in the Lunchroom located left of the maintenance shop.
