I authorize any references, churches, or other organizations listed in this application to give you any information they may have regarding my character and fitness for working with minors and I release all such references from liability for any damage that may result from furnishing such evaluations to the Home Church. I understand that any omission of material fact on this application may be grounds for rejection of this application. I also agree to submit "live scan" fingerprints for the purpose of doing a criminal background check through the California Department of Justice and the FBI. I hereby release said Police Department from any and all liability from such disclosure.
