    • □     I understand that there is an element of risk involved with the playing of all sports.

      □     I agree to abide by club guidelines at all times. I agree to follow the coaches and CLRC Board Members instructions without argument. I agree that this may be a verbal, visual, physical or demonstrative form of communication.

      □     I will ask for further clarification of any ambiguous and/or inaudible instructions.

      □     I understand and agree that I will be asked to leave the club session immediately if deemed to be deliberately not following the Codes of Conduct or ignore instructions from the Head Coach.

      □     I agree to this form being kept indefinitely by the Crystal Lake Rowing Club, on the understanding that the disclosed information will be kept confidential, and shared only between the Board of Directors and Coaches. Other than the club or where required in consultation with US Rowing or Crystal Lake Park District, the information on this form will not be supplied to any other third party.

      □     I will inform the coach if my circumstances change in the course of the season.

      □     Before each club session I will inform Coaches and relevant members of the club of any medication, allergies, injuries or other medical conditions which may affect my ability to participate fully in the sport on that day.

      Practice Policies

      □ Practice Attendance
      Attendance is extremely important for the performance of the boat and the team. If one member is not able to attend the boat lineup is not able to practice. Unlike many other sports your teammates depend on your attendance for the entire success of the boat.

      □ Missing Practice
      Please communicate your attendance to your coach, a lot of pre planning goes into boat line ups, the more advanced notice the better. Coaches will outline the best way they will have communication with rowers.

      □ Absences
      Excessive absences can result in:
      -Removal from current boat/seat.
      -Not allowed to row at practices.
      -Not allowed to row at regattas.

      □ Unexcused Absences
      Unexcused absences will be defined as not providing adequate notice of absence. Adequate notice will be considered more than 24 hours before the missed practice.

      If you miss more than three (3) practices in a season without an excuse, you forfeit your racing privileges for the season.

      If you miss five consecutive days without notifying a coach or administrator, The Crystal Lake Rowing Club will assume you have quit the team.

      Sickness and Injuries
      In case of suspected sickness or injury, an athlete should contact their coach and consult a physician or an appropriate doctor IMMEDIATELY.
      Upon taking a sick day(s), an athlete may be required to provide a doctor’s note.  Absence of such a note may prevent an athlete from participation in practice until cleared.

    • AUTHORIZATION TO USE PHOTOGRAPHS AND/OR AUDIO-VISUAL MATERIAL

      The Crystal Lake Rowing Club, hereafter referred to as the Corporation, to use, reproduce, and/or publish photographs and/or video that may pertain to my junior rower—including his/her image, likeness and/or voice without compensation. I understand that this material may be used in various publications, public affairs releases, recruitment materials, broadcast public service advertising (PSAs), or for other related endeavors. This material may also appear on the Corporation's Internet Web Page. This authorization is continuous and may only be withdrawn by my specific rescission of this authorization. Consequently, the Corporation may publish materials, use my child’s name, photograph, and/or make reference to him/her in any manner that the Corporation or project sponsor deems appropriate in order to promote/publicize rowing events.

      Assumption of Risk, Waiver of Claims and Indemnity Agreement

      CLRC’s provides a method whereby individuals interested in carpooling may voluntarily do so. Parents are welcome to travel with the team. Parents who agree to chaperone must adhere to the team method of travel and the team schedule. Users of the carpool are advised that its use is voluntary and entirely at their and their parent’s/guardian’s discretion. If parents/guardians do not wish to utilize the carpool it is their responsibility to get themselves and their rower to and from regattas. Rowers are not permitted to transport themselves or other rowers to regattas. No background checks of drivers or participants, driver record checks or insurance checks are undertaken by CLRC. CLRC does not make any assurances or certifications regarding the condition, suitability or safety of the vehicles being used by participants, the reliability or driving ability of participants or drivers, or whether those participants and drivers are insured.

       

      Responsibility for Insurance

      It is the responsibility of individuals participating in a carpool to notify their insurance provider of their intent to carry non-family passengers and to ensure that they have adequate coverage to protect both themselves and their passengers. It is the responsibility of the driver of the vehicle to ensure they have an acceptable driving record, are licensed accordingly and have adequate insurance to protect both themselves and passengers. CLRC is in no way responsible for verifying or certifying the reliability, driving ability or insurance status of drivers and participants.

       

      Waiver of Claims & Indemnification

      CLRC takes no responsibility for any personal injuries, deaths, property damages, financial losses or other damages or losses that may result from the use of carpooling. By participating in the CLRC’s carpool program, you hereby agree to waive any and all claims that you have or may have in the future against CLRC and to release them from any and all liability for loss, damage, expense or injury including death that you, your next of kin or a third party may suffer as a result of your participation in or involvement in the CLRC program. You hereby also agree to indemnify and hold harmless CLRC from any and all liability for personal injury, death, property damages and any other loss, and resulting claims or actions, arising from your participation in or involvement in the CLRC program.

      ROWING RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT PARENTAL CONSENT REQUIRED FOR ALL MINOR PARTICIPANTS (Less than 18 years of age)

       

      IN CONSIDERATION of being given the opportunity to participate in any way in the Crystal Lake Rowing Club. (hereafter "Club") activities ("Activity") until the term of which my dues have been paid, I, for myself, my personal representatives, assigns, heirs, and next of kin:

       

      1.   ACKNOWLEDGE, agree, and represent that I understand the nature of Rowing Activities, both on water and land based, and that I am qualified, in good health, and in proper physical condition to participate in such Activity; I further acknowledge that any questions I have concerning the Activity of the Club and its members has been fully explained to my satisfaction;

       

      2.   FULLY UNDERSTAND that: (a) ROWING ACTIVITIES INVOLVE RISKS AND DANGERS of serious bodily injury, including permanent disability, paralysis, and death ("Risks"); (b) these Risks and dangers may be caused by my own actions, or inactions, the actions or inactions of others participating in the Activity, the condition in which the Activity takes place, or the negligence of the Releasees names below; (c) there may be other risks and social and economic losses either not known to me or not readily foreseeable at this time; and I FULLY ACCEPT AND ASSUME ALL SUCH RISKS AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES I incur as a result of my participation in the Activity;

       

      3.   AGREE AND WARRANT that I will examine and inspect each Activity in which I take part as a member of the Club and that, if I observe any condition which I consider to be unacceptably hazardous or dangerous, I will notify the proper authority in charge of the Activity and will refuse to take part in the Activity until the condition has been corrected to my satisfaction;

       

      4.   HEREBY RELEASE, discharge, and covenant not to sue the Club, their administrators, directors, officers, volunteers, and other participants (each considered one of the Releasees herein) from all liability, claims, demands, losses, or damages on my account caused or alleged to be caused in whole or in part by the Releasees or, any other person or entity, or otherwise, including negligent rescue operations; and I further agree that if, despite this release and waiver of liability, assumption of risk, and indemnity agreement, I, or anyone on my behalf, makes a claim against the Releasees, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS each of the Releasees from any litigation expenses, attorney fees, loss, liability, damage, or cost which any may incur as the result of such claim.

       I have read this agreement, fully understand its terms, understand that I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature other than contained herein and intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law and agree that if any portion of this agreement is held to be invalid the balance, notwithstanding, shall continue in full force and effect.

      AND

      I, the minor's parent and/or legal guardian, understand the nature of rowing activities and the minor's experience and capabilities and believe the minor to be qualified to participate in such activity. I hereby release, discharge, covenant not to sue, and AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS each of the Releasees from all liability claims, demands, losses, or damages on the minor's account caused or alleged to be caused in whole or in part by the negligence of the Releasees or otherwise, including negligent rescue operations, and further agree that if, despite this release, I, the minor, or anyone on the minor's behalf makes a claim against any of the above Releasees, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS, each of the Releasees from any litigation expenses, attorney fees, loss liability, damage, or cost any may incur as the result of any such claim.

    • CLRC Safety Rules & Procedures

      1. All rowers/coxswains must pass a swim test & flip test.
      2. All rowers/coxswains must comply with instruction given by a Coach. 
      3. On water rowers will comply with the directions of their coxswain and coaches.
      4.  When the rowers are not able to go on the lake – it DOES NOT mean practice is or will be cancelled. There are alternative workouts or land practices. Land practices are just as important as water practices. Land practices allow for coaches to perfect rowing techniques, as well as to develop core muscles and to gain endurance.
      5.  At ALL TIMES, both in-season and off season, you will conduct yourself in a courteous, civil and proper manner as member of Crystal Lake Rowing. This includes at practices, at races, at school, on social media, and in any face-to-face or online communications. You are representing your team and your community. 
      6. Crew is a synchronized endurance team sport. Coaches will be looking for specific abilities, attitudes, qualities, and a 100% level of commitment. The absence of a high level of any of these traits could affect your position in a boat and on the team. Attendance at all practices is mandatory; it will be a major factor in compiling boat line ups. 

      Rowers Guidelines

      1. Everything you do and say must reflect favorably upon yourself, your teammates and the coaching staff. 
      2. You will treat everyone associated with this team with courtesy and the highest respect. Someone is always in earshot of you and you will be heard. 
      3. You must attend all volunteering activities approved by the board. The club depends on community support which means the club must also support the community.
      4. Cell phones are not permitted at practices. They must be kept in your bag & out of sight while at practice. 

      Rower Practice

      1. Rowers need to be at the boathouse at least 15 minutes prior to practice to get the launch in & get in the mindset for a productive practice.
        Rowers must wear clothes that will not hinder practice. Wearing shorts that get caught in the tracks or jackets where the oars get caught is not acceptable. 
      2. Rowes will keep talking in the boat to a minimum and at no time, there should be talking during a piece. 
        Crossfit practices at not optional. 
      3. You must let the coaches know one day in advance in order to have an excused absence. 
      4. Crew is a sport that consists of teamwork, commitment, and hard work. Rowing is a synchronized, endurance sport. Individual attitudes will not be tolerated with this sport. Teamwork is paramount. 
      5. All rowers will respect our equipment and practice areas. Water bottles will not be left around our practice areas. It doesn't matter who left the trash. 
        All rowers will show respect for each other, all coaches, and CLRC parents. 
        At the end of practice, jumpies are not optional. Jumpies may be given, but not limited to, the following: Late to practice, not signed into practice & showed up, teammate signing into practice but not showing up, talking in the boat, not being safe with the equipment, not being respectful to the teammates, complaining, getting clothes caught in the tracks
    • Swim Test Verification  
    • If your child has previously taken a comparable swim test (BSA swimming merit badge, lifeguard certification, SCUBA certification, etc.) you may complete the following in lieu of the below test.

    • Swimming Skills Test

    • Rowing is a water sport. The shells (boats) we row in are narrow and there is a possibility they can capsize. Because of this we require all our athletes (including coxswains) to be good swimmers. Below is the swimming ability test we require.

      Have a lifeguard, certified water safety instructor, or USRowing certified coach (if a coach is administering the test a lifeguard must be present) complete this form following your demonstration of skills.

      I.   Agreement

      □   An Athlete Rower Who is Not Completely Registered with CLRC

      If I am an athlete rower who is not yet completely registered because my account is not paid in full, or because I have incomplete paperwork or because of disciplinary action, or because I have not yet passed the required swim test, I am in agreement that, by signing this document, I will not hold CLRC liable for any injuries or accidents as may occur while I am traveling in one of the CLRC-approved launch boats with a CLRC-approved driver. I agree that I am not to be allowed to participate as a passenger in an CLRC-approved launch and I am no longer under CLRC's supervision if I refuse to sign this document. In that case, CLRC is not responsible for my health and well-being while being left unattended.

      □   An Athlete Rower Who is Completely Registered with CLRC

      If I am an athlete rower who through the course of the year might suffer from an injury or sickness and is unable to row for such reasons or has been selected by the coach to sit out and not row for a particular practice session or time period, and because I cannot be left at the boathouse unattended, I am in agreement that, by signing this document, I will not hold CLRC liable for any injuries or accidents as may occur while I am traveling in one of the CLRC-approved launch boats with a CLRC-approved driver. I agree that I am not to be allowed to participate as a passenger in an CLRC-approved launch and I am no longer under CLRC's supervision if I refuse to sign this document. In that case, CLRC is not responsible for my health and wellbeing while being left unattended.

      II.   In Addition

      The agreements above include any hour of the day during any part of the calendar year. By signing this document, I recognize that any person driving a launch on behalf of CLRC is not to be held liable as a representative for CLRC for any of the above-listed situations during any hour of the day during any part of the calendar year. 

    • Centegra Healthbridge Release of Liability  
    • On behalf of myself or the minor guest I am the parent or guardian of, I acknowledge that attendance at, use of, or participation in Centegra Health Bridge Fitness Center (CHBFC) facilities, activities, equipment or provided transportation could be dangerous and cause injury, including death, and is undertaken at the guest’s own risk. On behalf of myself and/or the minor guest I am the parent or guardian of, and my/our heirs, executors, and assigns, I fully waive, release and forever discharge Centegra Health Bridge Corporation, Centegra Health Fitness Center, LLC, and their respective affiliates, parents, subsidiaries, officers, directors, employees, agents, successors and assigns from any and all claims, damages, demands, rights of action, and causes of action, known or unknown, anticipated or unanticipated, arising out of or relating to attendance at, use of, or participation in CHFBC facilities, activities, equipment or provided transportation. My signature below verifies that I have read, understand and agreed to this waiver and release of any and all liability.
