1. We supply approximately a 10 foot by 10 foot SPACE ONLY located at Ellsworth Elementary 38454 N Carolina Ave, San Tan Valley, AZ 85140. If you require more space you must purchase a second booth.
2. You should bring a canopy in case of rain showers.
3. You must bring your own table and chairs, display booth, etc.
4. Set up starts at 8:00 am and you must be done setting up by 9:30 am. Clean up starts at 4p.m. when the event is over.
5. NO vehicles will be allowed to drive on school property (parking lot only ofcourse)
6. No bake sale items without proper Arizona Registration.
7. Please make your booth as professional as possible. This is a school event.
8. We ask that you donate an item, either a product or your service for raffles.
9. booth $30.00 ONLY ONE VENDOR PER BOOTH (NO Exceptions) (ONE BUSINESS)
10. No refunds will be given in the event of rain, wind, or event interruption or destruction of any kind. No Refunds at all.
11. Your application must be PAID before participating in the event.
12. If you need to make special arrangements due to handicap or other issue, please notify Carrie Ribeiro or Netty Riggs.
Jo Combs, San Tan Valley Home Garden Health & Lifestyle Event, and all affliates will not be held liable or responsible for your loss incurred by or at the event should there be a loss for any reason. Also, if you incur expenses for the school property, signing below will confirm that you will take financial responsibility. You also agree to carry your own business insurance and you participate at your own expense.
By signing and submitting this application, this indicates you agree with the terms above. YOU Also agree that if you do a Credit Card "chargeback" or dispute you will be charged an additional $30.00 ONTOP of your event Fee. YOU also agree that THIS is a legal agreement and can be given to your Credit Card Company and or Paypal as EXHIBIT A for proof of payment agreement.
