Thank you for your interest to serve at New Horizons Church. The Children/Youth Workers Policies must be read by all individuals desiring to be involved in any ministry related to Children and Youth (all children under 18 years of age) at New Horizons Church, for activities on campus or special events at other locations.

It is for the purpose of helping the New Horizons family provide a safe and secure environment for children and youth who participate in our various ministries. We trust that you share our desire to protect the children and youth whom God has entrusted to our care.

After reading the Children/Youth Workers Policies please read below and sign this form.
