By clicking the form below, you understand that submission of the form is subject to approval by your Faculty co-op advisor. Your faculty co-op advisor may contact you to provide additional information and/or recommend changes prior to approval.
If you are submitting a request to subsitute a study abroad experience for co-op, you also understand that you are required to provide a verification of completion of the study abroad expereince or a transcript from the global studies representative within the first two weeks of the semester immediately following the study abroad experience.
Furthermore, if you indicated that you will be requesting a change of employer for you next co-op rotation, you understand that you will need to complete and submit a Change of Assignment form (thorugh PAL) with your faculty co-op advisor.
Finally, registration for an EEP is subject to standard registration and billing procedures for co-op experiences.
