Letter of Agreement

• Your child’s Personal Trainer is responsible for scheduling all personal training sessions with you.
• Personal training sessions are to take place in your home, your building’s gym, virtually, or, with your written permission, outside in park.
   

Cancellation Policy

• If you need to cancel, please contact your Energetic Juniors Trainer first.
• If you have any difficulty reaching your Trainer, call Bonita Porte directly at: 212-879-1566 or e-mail: bonita@energeticjuniors.com
• Cancellation of a session - via e-mail or text to your child's trainer - must be received at least 24 hours in advance to avoid being charged.

Payment

• Payment for each program is due in full prior to the start of the first personal training session.
• All sessions are payable in advance. Check or Credit Cards are accepted.
• Sessions are non-refundable but can be used by another family member within six months.

Risks

I recognize that any exercise carries some risk to the musculoskeletal system and the cardiorespiratory system. I hereby certify that I know of no medical problem that my child has that would increase their risk of illness and injury as a result of participation in a regular exercise program. 

I understand that the Energetic Juniors independent contractor trainer adheres to the CDC recommendations of practicing social distancing and wearing face coverings. I attest that my child/teen and family have not been diagnosed with Coronavirus/Covid-19, and that we follow all CDC guidelines.

By signing this consent form I understand that I am personally responsible for my child’s actions during my tenure with Energetics, and that I waive the responsibility of this organization if my child should incur any injury.
