This form will need to be submitted once. If you have not performed with UNIPAC within the last two years, you will need to resubmit the agreement.

This agreement is made between the Utah/Idaho Performing Arts Company, under the direction of Joey Calkins, hereafter referred to as UNIPAC and (name entered above), hereafter referred to as PERFORMER. This online form replaces the previously supplied "paper" or "hard copy" versions. For the sake of simplicity "PRODUCTION" refers to concerts, recitals, and staged productions.

By submitting this form, PERFORMER agrees to the following:
PERFORMER understands his/her appearance in PRODUCTION is gratis. *

PERFORMER will maintain a professional attitude at all times. *

PERFORMER agrees to indemnify and hold UNIPAC blameless for injury or loss of life or limb resulting from PERFORMER’S actions. *

PERFORMER gives permission for UNIPAC to use his/her name, image, and/or likeness in all marketing for PRODUCTION and for future marketing and archival purposes for UNIPAC. *

PERFORMER will complete this agreement and the Emergency Contact Form (not required for concerts/recitals) no later than the provided due date. PERFORMER understands his/her role may be recast if agreement is not completed by the due date. *

PERFORMER agrees to attend rehearsals as scheduled unless other arrangements are made with the shows director or concert producer. *

These terms are subject to be altered upon mutual agreement between UNIPAC and PERFORMER. Other duties, responsibilities, or expectations of PERFORMER and/or UNIPAC not listed here or in the Season Packet will be discussed and mutually agreed upon by both parties.
