WindBand Installation Verification Form

Overview

This form must be completed and submitted as part of the WindBand installation. Carefully follow the WindBand Installation Verification Form (IVF) Instructions supplied with each order of WindBands or see them at this support page link: https://bit.ly/2vmDsjg.

The Installation Inspection Form (IVF) requires detailed photographs that can only be taken from the crane basket or aerial work platform (man lift). Do not leave the job site until the necessary photographs have been acquired. Some tower owners and/or tenants require an approved WindBand Installation Inspection form before payment to installing contractors is made. After the inspection report is submitted using this online form it will be reviewed by WindSwept Designs USA. Results will be returned to the installing contractor and tower owner.

It is not difficult to take suitable photos if the installer is careful and understands what images are needed. Photos must show a high level of detail, be in focus, and include the required views of the cell tower and WindBands as specified. Failure to do so will result in a rejection of the Installation Report and may affect payment to the contractor. Carefully review the pictures before leaving the crane basket or manlift. Go back and retake them if you have any doubts.
