We know many families have two working adults and we don't expect anyone to be a super hero and quit a day job just to be able to take home a puppy. Every puppy family has different needs, and this information form helps us identify who needs what information and what homes might make the best fit. If you're a veteran small breed dog owner, we don't want to waste your time regaling things you already know; by the same token, if this is your first puppy or first small breed dog, we want to make sure you leave prepared for what life with a puppy may bring. This form along with the sales contract/Health guarantee must be submitted prior to picking up a puppy. We show puppies by appointment only!! PLEASE NOTE:THIS IS NOT THE FINANCING APPLICATION

Please tell us a little about where puppy will live as a puppy (we know circumstances change).

Completing this application is a step in the process to acquiring a puppy from us, but it does not guarantee I'll get a puppy from this or any future litter. While it's unlikely, Dannys Family Pets reserves the right to refuse any placement at any time.

YOUR PICK OF PUPPY: Puppies will be assigned in deposit order by the first or second preference of the depositor as indicated on the application. If neither of your choices are available, you may elect another puppy from the litter. Should you elect to not take a puppy from the litter, the deposit is forfeited and the deposit applied to the costs of the deposit applied to finding the puppy another home. It is not forwarded to an upcoming litter. You should review both the deposit policies on our website for any updates.

GOING HOME: We cannot hold puppies for extended times; it is critical for their social development that the litter departs within a reasonable timeframe. After 48 hours we charge a boarding fee of $45 a day. If you have a big vacation planned or have events that would conflict with a young puppy, please know puppies cannot be boarded in a kennel until they have received their full immunizations and their rabies vaccination which cannot be done until 3 months of age.

CONTRACT: We have a contract that will be emailed over to you and will not place a puppy without a signed contract. We believe it's the professional thing to do to memorialize our obligations to each other on paper so there are no questions later, and for that reason it's a bit lengthy. If you have questions about specific terms, you need to contact us prior to picking up your puppy.
