It is the policy of Angel Healthcare Staffing to comply with all applicable laws related to promoting a drug-free workplace and require all employees to report to work able, to perform their jobs in a satisfactory manner. The company conducts drug and alcohol screening accordingly. While on the company’s premises and/ or while conducting business for the company, no employee may manufacture, use, possess, distribute, sell or be under the influence of or engage in the unlawful manufacture, distribution, dispensation, possession or use of illegal drugs or alcohol.
The company requires and only allows drug and/or alcohol test for:
- Upon Hire – all new and rehired/re-activated employees are required to provide a urine sample for drug testing prior to assuming any position. Upon hire testing will be completed after acceptance of a contingent offer of employment is made.
- Reasonable Suspicion/Probable Cause – In instances where illicit drug and/or alcohol use is suspected based on reasonable suspicion/probable cause an employee may be required to submit to a drug and/or alcohol test.
- Post-Accident – All employees involved in an on the job injury which arises under circumstance that suggest possible involvement or influence of drugs and/or alcohol in the accident or injury event will be required to submit to a drug and/or alcohol test.
As part of the testing procedure the company tests for sample adulteration/tampering and sample dilution. Subject to any limitations imposed by law, a refusal to provide a urine sample under the conditions set forth in this policy may result in disciplinary action up to and including termination.
The legal use of prescribed drugs is permitted on the job only if it does not impair an employee’s ability to perform the essential functions of the job in the manner that does not endanger them or any other individuals in the workplace.
Consent
I give my consent to the company to submit to a drug or alcohol test. I hereby allow the company to take necessary specimens of urine, blood, hair, breath, etc. from me to test of any controlled substance or alcohol in violation of the company Drug Free Workplace Policy. I authorize the laboratory or medical personnel retained by the company to administer these tests to release an and all results from the testing procedures which include any and all documentation plus any other type or oral or written communication. I also hereby release and hold harmless the company, the laboratory, any physicians or medical personnel involved in the testing procedure and any and all employees, directors, officers, and successors of the company from any liabilities, claims, and cause of action, know or unknown, contingent or fixed, that may result from this drug screening/test.
