DAYCARE/BOARDING EVALUATION APPOINTMENT FOR NEW CLIENTS

EVALUATIONS ARE REQUIRED FOR ALL NEW DOGS OVER THE AGE OF 6 MONTHS BEFORE ATTENDING DAYCARE OR BOARDING AT DOG GONE SMART.
Please Note

(1) You can reserve just ONE evaluation appointment slot for MULTIPLE dogs

in the same family (UP TO 3 DOGS) .

YOU MUST LIST ALL DOGS WITHIN THIS FORM

(2) If your dog is UNDER 6 MONTHS it does NOT require an evaluation but still REQUIRES a completed PET PROFILE FORM

Daycare/Boarding Evaluations are Meet and Greets for new dogs to establish comfort and evaluate how they will acclimate to our play environment. They take about 15 minutes and if approved you will be eligible to begin attending Daycare and Boarding at Dog Gone Smart. If your dog needs a little more support to start out in such a playful social environment, we will be able to set you up with a socialiazation coach to help set them up for success.
If you have filled out your PET PROFILE FORM and just need to UPLOAD VACCINES pleases do so in our VACCINE UPLOAD PORTAL. We must have your vaccines before you arrive for your appointment. Thanks and we look forward to meeting you and your dog!!
