The information provided on this form will be used by the Ultimate Sports Association (USA) officials to assist you in the search/awarding of scholarships.

USA uses the funds in its scholarship fund, provided by generous community donors, to provide scholarships for registration fees to children, who without financial assistance would be unable to participate in USA and its affiliate's sports programs.

USA is a 501(c)(3) non-profit organization with limited funding available for scholarships.

No guarantee of assistance is implied by this application. If the number of scholarship applications submitted and approved exceeds the amount available, the scholarships will be awarded by a lottery system.

Scholarship applications are awarded on a rolling basis and as funds are available.
