The form will NOT be submitted until you complete all the pages and sign/submit the form. Be reminded that your chapter's calendar and associated activities must be submitted AND approved two weeks before recruitment begins!

Please provide as much information as you can. We recognize that you may not have exact dates yet and that is okay. Simply use the first day of the week that you plan on having the event or lesson.

Please list the date(s) and details of any activities not contained within the Road to Wisdom. If none, please put "N/A".
All activities not contained within the Road to Wisdom (e.g., chapter-specific ceremonies, additional MC requirements, etc.) should be submitted through the Activity Review Form (see below).
Please submit more specific information for the activities listed above by completing the Activity Review Form. A form should be submitted for ALL activities not explicitly outlined or referenced in the Road to Wisdom. Questions can be emailed to the Membership & Expansion Coordinator at thomas@kkytbs.org.
If there are any gaps between longer than one (1) week in your New Membership Education Process, please list the reason(s) below (e.g. Fall Break, Band Trip, University Policies, etc.).
