    This form must be completed prior to any recruitment or membership education activities taking place in a given semester. The form will NOT be submitted until you complete all the pages and sign/submit the form. Be reminded that your chapter's calendar and associated activities must be submitted AND approved two weeks before recruitment begins!

    You are also acknowledging that your DOB and Sponsor have reviewed and approved your Chapter Calendar prior to completing this form.

     

    Finally, you understand that your chapter may not start your recruitment/Membership Education Process until your Chapter Calendar and Activity Reviews have been approved. You also understand that starting Recruitment/Membership Education Process without proper approval of the Chapter Calendar/Activity Reviews could result in disciplinary action being taken against your chapter.
