This scholarship celebrates the life and passion of Susan Madsen who never failed to be on a soccer field encouraging, managing, cheering and supporting all around her.  Susan was an active member of Utah Youth Soccer who gave countless hours of support and service to the Utah Soccer Community, always unconditionally and with a laugh.  This scholarship is made possible through contributions to the Susan K Madsen Memorial fund by her family, friends and the Utah Soccer Community.

• The Susan K Madsen Youth Service Scholarship awards $1,500 to a graduating senior to be used one time toward expenses to an accredited college, university, community college or tech school. A scholarship will be awarded to one male and one female.
• Applicants must be a member of Utah Youth Soccer Association and a graduating senior. You can also apply for the UYSA College Scholarship. Applicants will only be awarded one of the two scholarships.
• Successful applicants are notified after the selection process is complete and will be recognized at a formal awards presentation during the Annual UYSA Awards Gala. Recipient of the scholarship will receive a seat (plus two) for the Awards Gala. Attendance is required to receive the award. More seats can be purchased online or by calling the UYSA office.
• The Scholarship is contingent upon proof of acceptance and enrollment in a 2-4 year accredited college, university, community college or Tech School with a minimum course load of 12 credits/units (full time status). College soccer participation is not needed.

Applicants must submit the following to the Utah Youth Soccer Association and must be received at the address listed below NO LATER THAN September 29th, 2024 at 5:00 pm (End of Business).

• A completed application form
• A digital Photo – Winners will be featured on the UYSA website.
• A copy of your official high school transcript (including ACT and/or SAT scores.  (A 3.0 grade point average (GPA) minimum is needed to apply for the scholarship.)
• Two letters of recommendation may be from the following: coach, community leader, counselor, teacher, principal or peer. A phone number and email address of the person (s) completing the letters of recommendation should be included in the letter(s).
• Written Essay – no more than 500 words, tell us about how service has helped you grow as a person.
• 3-5 minute video (uploaded on YouTube) with the following information - Title: Madsen Scholarship: (Your last name).
