• Naked Kisses Dog Rescue Adoption Application

  • Please be as thorough as possible when completing the application in order to ensure your application is processed in a timely manner. Our rescue dogs are not adopted on a first come, first serve basis, but to the family who will be the best match.  Since all of our dogs are in foster homes, we set up meet and greets only once there is an approved application on file.

    Adoption fees are used to recoup a portion of the veterinary expenses that our rescue incurs on incoming dogs.  Please remember that your paid adoption fee to NaKiD Rescue may be tax deductible; consult with your tax preparer.

    We do not maintain a waiting list for adoptions.  If there is not a dog available that you are interested in, please hold off on completing an application.

    Before submitting your application for one of our rescue dogs, please understand that it is the responsibility of the approved adopter to arrange transportation. Arrangements must be approved by our rescue prior to any adoption being final. We do not have the resources to assist with transporting an adopted dog to you, so please plan to travel to the foster dog in order to pick it up and take it home. Some dogs may not qualify for airline travel and can only be transported by automobiles.

  • The first step in our application screening process is a detailed vet reference check. Please let your vet know we will be contacting them to verify all your pets currently meet the following requirements:

  • Are your dogs up to date on annual heartworm testing?*
  • Are your dogs on monthly heartworm prevention?*
  • Are your pets up to date on state required vaccines (ex: rabies)?*

  • Are you familiar with crate training?*
  • Are you willing to house train a dog?*
  • Have you ever house trained a dog or puppy before?*
  • Are you willing to work through your new pet's issues, if any?*
  • Are you aware that a new environment is stressful for your pet, and they may exhibit uncharacteristic behavior?*
  • Do you understand that a dog can be an 18 year commitment, and can cost a minimum of $300 - $500 per year to care for?*
  • Are you financially willing and able to cover the costs of supplies, quality dog food, grooming, routine vet care, and unexpected illnesses or surgeries?*
  • Do you agree to having a rescue volunteer conduct a home visit or your submitting a detailed video walkthrough for a pre-adoption visit and/or a follow up adoption visit?*
  • Do you object to providing detailed updates, including photos, 1 month, 3 months, 6 months, and 12 months after adoption?*
  • Have you researched and do you understand possible medical issues that Chinese Cresteds can have?*

  • If approved, we require the signed Adoption Agreement and the adoption fee to be returned to the rescue within 24 hours of official email approval. This is to secure the adoption of the dog, otherwise we will continue processing other applicants for the dog. Is this something you are willing and able to do if you are approved?*
  • I understand that Naked Kisses Dog Rescue (NaKiD Rescue) has the right to deny any adoption, for any reason. By completing this application and signing below, I certify that I am at least 21 years of age and will assume all responsibility for the care and well-being for the dog that I am adopting through NaKiD Rescue.  I further agree that I will be contractually bound to:

    ·       Take my dog to the vet at least once annually for a physical exam, heartworm testing/preventative, and any shots required or suggested by the veterinarian and/or local/state regulations.

    ·       Take my dog to the vet in a timely manner if it is ill.

    ·       Maintain proper care, including routine dental care, heartworm prevention, any necessary flea/tick treatments.

    ·       Return this dog at my own expense to NaKiD Rescue, if for any reason I feel that I am unwilling or unable to continue caring for this dog in a loving and humane manner. Two weeks notice must be given to arrange to take the dog back.
