In connection with my application for internship and as a condition of continuing internship, I understand that investigative background inquiries may be made on me, including previous employers and schools. These reports will include information as to my character, work habits, performance, education, compensation, and experience, along with reasons for my termination of internship from previous employers. I authorize, without reservation, any party or agency contacted to furnish the above mentioned information and release all parties involved from liability and resonsibility for doing so. I hereby consent to obtaining the above information from Christ the King Community Church and/or any of their agents. This authorization and consent shall be valid in original, fax, or copy form.

As an intern at CTK, the intern is not entitled to a job at the conclusion of the internship. Christ The King and the intern understand that the intern is not entitled to wages for the time spent in the internship, unless previously agreed as a paid internship.
