Please fill out for your pet each time they board with us. If boarding more than one pet, please submit a separate form for each pet. Drop off and pick up Monday through Friday is between 8 am - 12 pm and 4:00 pm - 6:00 pm. Saturday is between 8 am and 11:30 am. Sunday is prepaid pick up between 10 am - 11 am only.

I have disclosed all known medical conditions, injuries, illnesses, allergies, medications, dietary restrictions, mobility limitations, behavioral concerns, prior incidents of aggression, bite history, escape tendencies, and any other information that may affect the health, safety, handling, supervision, or care of my dog while boarding. I understand that failure to provide complete and accurate information may increase the risk of injury, illness, stress, escape, or other harm to my dog, other animals, facility staff, or members of the public. I agree to promptly notify the boarding facility of any change in my dog's health, behavior, or medical condition that occurs before or during the boarding period. I authorize the boarding facility to seek veterinary care for my dog in the event of an illness, injury, or emergency when I cannot be reached within a reasonable period of time. They are to use all reasonable precautions against illness, injury, or escape of my pet, but they will not be held liable or responsible in any manner what so ever, under any circumstances, on account of the care, treatment, or safe keeping of my pet, as it is thoroughly understood that I assume all risks. I understand that reasonable efforts will be made to contact me or my designated emergency contact before treatment is authorized, except when immediate veterinary attention is necessary to prevent suffering, serious injury, or death. I acknowledge that the boarding facility has reviewed the information provided and may rely upon my disclosures in determining appropriate housing, supervision, handling, exercise, and care for my dog. Should the circumstance arise that my pet remains unclaimed after the date which I have stated as the pick-up date, I understand that written notice will be mailed to the address on file. Seven days after I understand that such action will not relieve me from paying all costs of your service and the use of your hospital, including the cost of the boarding service. Knowingly providing false or materially incomplete information on this form may be considered evidence in any civil or administrative proceeding arising from an incident involving the dog during the boarding period.
