  • Verification Report opens in TX-UNPS November 7, 2018.

    The SFA Verification Report in TX-UNPS is due February 1, 2019.

  • Instructions: Use this practice verification template to collect all the data necessary to complete the Verification Report before it opens in TX-UNPS on November 7th. 

    This practice report will provide warnings and helpful tips that will help you complete the actual report without any errors that lead to possible findings and corrective actions.

  • Instructions:

  • ANNUALLY, each SFA, including ALL RCCIs, with schools operating the National School Lunch Program (NSLP) and/or School Breakfast Program (SBP) must report verification information. All SFAs, including SFAs with all schools exempt from verification requirements, must complete applicable sections.

     

    NOTE: SFAs that are Special Milk Only are exempt from filing an SFA Verification Collection Report.

  • Section 1 - Total Schools, Residential Child Care Institutions (RCCIs), and Enrolled Students

  • All SFAs must report Section 1. Report schools or institutions operating the NSLP and/or SBP as of the last operating day in October. 

  • 1-1 Total Schools (Do not include RCCIs):

    1-2 Total RCCIs (Do not include schools counted in 1-1):

  • 1-2a RCCIs with Day Students (Report only day students in 1-2aB)

  • ERROR: The number of RCCIs with day students cannot be greater than the total number of RCCIS. Please correct the numbers in box 9 and box 7. 
  • 1-2b RCCIs with NO Day Students

  • ERROR: Check the number of RCCIs entered. 

    Box 9 + Box 11 cannot be greater than Box 7

    If there is only 1 institution that has both day and residential students, enter only the information for the day students.

     

  •  ERROR: Check the number of students entered. 

    Box 10 + Box 12 cannot be greater Box 8

    If there is only 1 institution with both day and residential students, enter only the information for the day students. 

  • Cannot report students in zero schools and/or RCCIs. Please correct the numbers in Section 1.
  • Section 2 - SFAs with schools operating alternate provisions

  • Only SFAs with alternate provisions must report Section 2. Report schools or institutions operating the NSLP and/or SBP as of the last operating day in October. 

  •  

    ERROR: The number of FREE plus  REDUCED PRICE students in schools operating Provision 2/3 in a NON-BASE year for NSLP and SBP cannot be greater than the total number of students in those schools. Please correct the numbers in fields 2-2a B, 2-2b B, and/or 2-2 B. (fields 16, 17 and 18) 
  •  

    ERROR: The number of FREE students in schools operating Provision 2/3 in a NON-BASE year for NSLP and SBP cannot be greater than the total number of students in those schools. Please correct the numbers in fields 2-2a.B (box 17) and 2-2.B (box 16). 
  •  

    ERROR: The number of REDUCED PRICE  students in schools operating Provision 2/3 in a NON-BASE year for NSLP and SBP cannot be greater than the total number of students in those schools. Please correct the numbers in fields 2-2a.B (field  18) and 2-2.B (field 16). 
  •  

    ERROR:  RCCIs are not eligible to elect CEP. Please correct the numbers reported in 1-2A and/or 2-3A (fields 7 and 19)
  • Note: 2-4 and 2-5 are read-only because they are not applicable, but are still on the Verification Report. 

    If you do operate an alternative provision program for only SBP or NSLP, please contact your regional ESC for instructions. 

  • Section 3 - Students approved as FREE eligible NOT subject to verification

  • All SFAs must report Section 3 or check box 3-1 if applicable. Report students approved FREE eligible as of the last operating day in October. 

  • The following CEs will check box 3-1. Checking the box "turns off" section 3. 

    • RCCIs with no day students
    • RCCIs with day students that remain enrolled at the home district
    • CEs that all sites participate in either CEP or P2

    Check the box only if ALL schools and/or RCCIs in the SFA were not required to perform direct certification with SNAP (i.e. NON BASE year Provision 2/3 or CEP for all sites). 

  •  

    You have reported that all of the sites in your SFA are operating either Provision 2 or CEP. Please check the box in field 3-1 and 5-1. Note: Matching for the purposes of CEP is not considered Direct Certification for the purposes of reporting on this form. Direct Certification results for Provision 2 and CEP are reported on the Direct Certification/Medicaid Report. 

    The fields for reporting the number of applications in Section 4 and the results of verification in Section 5 have been disabled on this practice template since you will not report those results. You will not report any data in those fields in the actual SFA Verification Report in TX-UNPS. 

  • 3-2 Students directly certified through Supplemental Nutrition Assistance Program (SNAP):

  • You have indicated that you were not required to do perform direct certification (3-1 - checkbox 21). If this is not correct and you need to report the number of students directly certified with SNAP at sites not participating in CEP or Provision 2, uncheck 3-1 (checkbox 21).  

     

  • Report students directly certified through other programs in 3-3. This includes

    • Temporary Assistance for Needy Families (TANF)
    • Medicaid-Free
    • Food Distribution Program on Indian Reservations (FDPIR)
    • Documented Homeless
    • Documented Migrant
    • Documented Runaway
    • Documented Foster
    • Head Start
    • Pre-K Even Start
    • Non-Applicant but approved by local officials 

    DO NOT include SNAP students already reported in 3-2.

    DO NOT include directly certified MEDICAID Reduced Price.  

  • 3-3 Students directly certified through other programs: 

  • ERROR: The number of students directly certified with SNAP or other programs cannot exceed the total number of students in the SFA. Please correct the numbers in the fields 1-1B, 1-2B, 3-2 and/or 3-3 (fields 6,8,22 and 23).
  • 3-4 Students certified categorically FREE eligible through SNAP letter method.

  • Section 4 - Students approved as FREE or REDUCED PRICE eligible through a household application

  • You have reported that all schools in your SFA are in a non-base year for Provision 2 or CEP and therefore will not have any approved applications. Please check the boxes in field 3-1 (checkbox 21) and 5-1 (checkbox 34) and leave the rest of the fields in Sections 3, 4 and 5 as blank. 
  • ALL SFA collecting applications must report Section 4. Report number of applications (A) approved as of October 1st. Report number of students (B) as of the last operating day of October. 

  • 4-1.  Approved as categorically FREE Eligible. Based on those providing documentation (e.g. a case number of SNAP, TANF, FDPIR on an application)

  • Warning: You have reported {2641b214} approved students, but 0 applications. Please correct the number of applications.   
  • The number of applications does not change if the students' eligibility changes- e.g. if a student on a categorically eligible application is directly certified during the month of October, the number of applications will remain the same, but the number of students will be only those that are on the application as of October 31st. It is possible to have fewer students than applications. 

  • Warning: The number of students approved as categorically FREE eligible based on application (4-1 B - field 26) and by direct certification with programs other than SNAP (3-3 field 23) is the same.  Please confirm that the number is Sections 3 and 4 are correct. Note: Do not report the same students in both sections. 
  • Warning: The number of students approved as categorically FREE eligible based on application (4-1B field 26) and by direct certification with SNAP (3-2 field 22) is the same. Please confirm that the numbers in Section 3 and 4 are correct. Note: Do not report the same students in both sections. 

     

  • Warning: You are reporting an average of 6+ students per application for the categorically eligible FREE applications. Even with the different reference dates for these two fields (Oct 1 vs. Oct 31), this is unexpectedly high. Please correct the number of students and applications reported in fields 4-1 A (field 25) and or 4-1 B (field 26). Note: Do not report the same students in both sections 3 and 4. 
  • 4-2 Approved as FREE Eligible. Based on household size and income information.

Even with the different reference dates for these two fields (Oct 1 vs. Oct 31), this is unexpectedly high. Please correct the number of students and applications reported in fields 4-2 A (field 27) and or 4-2 B (field 28). Note: Do not report the same students in both sections 3 and 4. 
  • 4-3 Approved as REDUCED PRICE Eligible. Based on household size and income information. 

  • Warning: You have reported {3043b}approved students, but 0 applications. Please correct the number of applications.   
  • Warning: You are reporting an average of 6+ students per application for the categorically eligible FREE applications. Even with the different reference dates for these two fields (Oct 1 vs. Oct 31), this is unexpectedly high. Please correct the number of students and applications reported in fields 4-3 A (field 29) and or 4-3 B (field 30). Note: Do not report the same students in both Sections 3 and 4. Medicaid Reduced-Price direct certified student count is reported on the Direct Certification Medicaid Report, not the SFA Verification Report. 
  • Warning: The number of students is equal to the number of applications. Please confirm that the numbers of students and applications reported are correct.  
  • Total number of approved applications: (This is not on the Verification Report, but it is the number that is used to calculate the sample size for verification in the Standard Method and Alternate Method 1).

  • ERROR: The number of approved applications cannot be greater than the total number of students in the SFA. Please correct the numbers in Section 1 and 4 so that 1-1B + 1-2B is greater than 4-1A +4-2A + 4-3A. (Field 6 + field 8 > field 25+field 27+field 29)
  • ERROR: The number of students in special provision schools, plus the number of students certified as FREE through direct certification, plus the number of students approved by an application cannot be greater than the total number of students in the SFA. Please correct the numbers so that 1-1B + 1-2B is greater than or equal to 2-2B + 2-3B + 3-2B + 3-3B + 4-1B + 4-2B +4-3B. (Fields 6 + 8 is greater or equal to fields 14+16+20+22+23+26+28+30)

    The number of students with free and reduced-price eligibility includes the count of students from: 

    • 2-2 Provision 2/3 Non-base year (2-2 a and 2-2b)
    • 2-3 CEP (2-3b)
    • 3-2 DC-SNAP
    • 3-3 DC-Other
    • 4-1B. Categorically Free Eligible
    • 4-2B. Free Eligible
    • 4-3B. Reduced-Price Eligible

    Please check the numbers in those fields to make sure that you are not double reporting students.

     

  • Section 5

  • Instructions

    • Schools/RCCIs in which all children have been certified under direct certification procedures including children documented as eligible foster, migrant, runaway or homeless children.
    • RCCIs which do not have day students.
    • Schools electing the Community Eligibility Option.
    • Schools/RCCIs in which FNS has approved universal meal service through census data or using socioeconomic surveys.
      • e.g., special cash assistance claims based on economic statistics regarding per capita income (Puerto Rico and the Virgin Islands).
    • Schools participating only in the Special Milk Program.
    • Schools in which all children are served with no separate charge for food service and no special cash assistance is claimed, (i.e., non-pricing programs claiming only the paid rate of reimbursement).
    • All schools are Provision 2/3 schools in a non base year.
    • Schools which do not have any free or reduced price eligible students.
    • Other FNS determined exemptions on a case-by-case basis.
  •  

    You have reported that all of the sites in your SFA are operating either Provision 2 or CEP. Please check the box in field 3-1 and 5-1. Note: Matching for the purposes of CEP is not considered Direct Certification for the purposes of reporting on this form. Direct Certification results for Provision 2 and CEP are reported on the Direct Certification/Medicaid Report. 

    The fields for reporting the number of applications in Section 4 and the results of verification in Section 5 have been disabled on this practice template since you will not report those results. You will not report any data in those fields in the actual SFA Verification Report in TX-UNPS. 

  • "No" is pre-selected when the report is submitted in TX-UNPS. Don't skip this one. 

  • The number of applications entered for the verification sample is larger than the total number of approved applications. Please check the number entered in 5-5 (field 38). 
  • Based on the number of applications you entered in Section 4, this is the sample size for the verification method you chose. If there is a discrepancy, check that the number of applications entered was the number of approved applications as of October 1st. 

  • All SFAs must report 5-7 or check box 5-6 if applicable. 

  • If 5-6 is checked, skip 5-7. 

  • 5-7 Confirmed through direct verification: Report if FREE and/or Reduced price eligibility is confirmed through direct verification with SNAP/TANF/FDPIR as of November 15th.

    If direct verification was not performed, then check box 5-6 (field 39).

  • 5-8 Results of Verification by Original Benefit Type

    For each original benefit type (A, B & C) report the number of applications and students as of November 15th for each category (1, 2, 3, & 4). Do NOT include students and applications already reported in 5-7A or 5-7 B (Direct verification applications and students). 

    DO include the results of any applications verified for cause.

  • The number of applications verified is greater than the total number of applications approved. Please check the numbers of applications in Section 5-8 and Section 4-1, 4-2 and 4-3. 
  • The number of categorically-free applications verified is greater than the total number of applications approved. Please check the numbers of applications in Section 5-8 and Section 4-1. 
  • The number of income-based free applications verified is greater than the total number of applications approved. Please check the numbers of applications in Section 5-8 and Section 4-2. 
  • The number of reduced-price applications verified is greater than the total number of applications approved. Please check the numbers of applications in Section 5-8 and Section 4-3. 
  • The number of applications verified is greater than the total number of students. Please check the numbers of applications in Section 5-8. 
  • The number of applications certified and verified between Oct 1 and Nov 15 should equal the number of applications in the verification sample and those verified for cause.
