The date requested will be reserved when Houston Flashy Photo Booth receives a completed rental agreement and a $200 deposit. 30 days prior to the date of the event the rest of the rental is due. Payment can be paid by cash/credit card deposit is refundable for 7 days. Your event WILL NOT be added to our calendar until this form is signed. The following contract and its terms will set forth an agreement between Houston Flashy Photo Booth (Provider) and(Client: see above) the parties, for an event taking place at (Event address:See Above) This written contract sets forth the full, written intention of both parties and supersedes all other written and/or oral agreements between the parties. The service period will be from (See Above form) AGREEMENT Provider agrees to have a photo booth during this period; but occasionally, operations may need to be interrupted for maintenance of the photo booth. Event location must be accessible for photo booth delivery & setup 1 hour before the start of event. ACCESS, SPACE & POWER FOR PHOTO BOOTH WE DO NOT SET UP OUTSIDE or UNDER A TENT. PREPARATION Client will pre-arrange for an appropriate space for and access to the photo booth tower at event's venue. Accessibility for the photo booth delivery will be available 45 mins to 1 hour before the event. All photo booths require a space 10 feet by 10 feet by 8 feet high for each booth tower. Make sure there are no obstacles that will make delivery and setting up of the photo booth unachievable. Electrical requirement of a standard 120V electrical outlet within 25 feet of desired location for each photo booth operation. EQUIPMENT &SAFETY Client acknowledges that he/she shall be responsible for any damage or loss to the provider's equipment caused by any misuse by client or its guests or any theft or disaster. Children under 12 years of age need to be accompanied by a parent. BOOKING &CANCELLATIONS Client acknowledges that Houston Flashy Photo Booth requires at least a $200 booking deposit for booth rental to secure the date, through check, cash or credit card. The remaining needs to be paid 30 days before the event. Bookings made within 30 days of event must be paid in full. Any request for a date change must be made in writing/email in advance of the original event date as soon as possible. Change is subject to Houston Flashy Photo Booth availability and receipt of a new photo booth service contract. If client shall cancel within sixty (60) days of the event date, a $200 or 50% cancellation fees will be deducted from the amount paid. If client shall cancel event within fifteen (15) days of the event date, the full amount paid will be forfeited to Houston Flashy Photo Booth. MODEL RELEASE All photos are property of Houston Flashy Photo Booth. The commercial use of the photos without proper acknowledgement/permission from Houston Flashy Photo Booth is prohibited. Houston Flashy Photo Booth reserves the right to use and publish images for purpose of display, portfolio, &website. Photos may appear on website homepage slideshow. MISCELLANEOUS TERMS If any provision of these terms shall be unlawful, void, or for any reason unenforceable under Contract Law,then that provision shall be deemed separate and shall not affect the validity and enforce ability.
