    We're sad to see you leave your Auto-Renew Membership, however hope you still practice with us when your schedule allows.
  • Before you submit your cancellation request, here's some information for your consideration so you can make the best decision for you based on your practice habits & lifestyle:

    • Our prices increase from time to time, however if you maintain your current Auto-Renew Membership with us, your price stays the same. Over time, this can create significant savings compared to the price of Drop Ins or Class Cards.
    • If you anticipate a temporary reduced practice schedule, please email us to inquire about your hold entitlement. This might offer you the best of both worlds - a short-term alternative that keeps your long-term advantage! :)

    To initiate your request, please fill out & submit the form below (you will receive a copy of this form once submitted). Once your cancellation is complete, you will receive an email confirmation within 5 business days. Should you have any questions, please email us in advance to clarify.

    Auto-Renew Memberships are ongoing monthly contracts, automatically renewed unless 15 days written notice of cancellation request is provided.

    • If minimum contract commitment has been fulfilled, you may cancel any time by providing at least 15 days advance notice before your next payment (e.g., if your next charge comes out December 15th, you must submit this form by December 1st)
    • If minimum contract commitment has not been fulfilled, early cancellation fees apply as outlined in your contract.

     

  • Cancellation & Final Practice Date:

    Once your final payment goes through you'll have another 30 days to practice to your heart's content!
