The purpose of this form is to streamline the inquiry/complaint process about an issue within the NE LSC. Upon receiving your form, the Admin Review Chair will be able to assess the response needed and will contact you.

Any issue of sexual misconduct or abuse must be reported directly to USA Swimming: LINK TO ONLINE REPORTING FORM.

Any issue of bullying should be taken up with the coach of the team. If that has already been addressed and no action taken, feel free to submit this form for further review.
