I understand that if I am hired by the organization, this authorization shall remain in full effect throughout the duration of my employment.
I authorize all persons, schools, companies and law enforcement authorities to provide any information concerning my background and hereby release any said person, school, company and law enforcement authority from any liability or damage whatsoever, for providing this information.
I understand that the use of illegal drugs is prohibited during employment and if company policy requires I am willing to submit to drug testing to detect the use of illegal drugs prior to and during employment.
I understand that this application form is intended for use in evaluating my suitability for employment and it is not an employment contract. I also understand that no verbal promises or guarantees relating to employment are binding upon the company and that, if employed, I will be an employee "at will" and may be discharged at any time, for any reason. I further understand that no one is authorized to alter that at-will notice of employment, unless it is signed in writing by the President of Gorman Heritage Farm.
Gorman Heritage Farm acknowledges the special nature and value of full-time residential staff members and seeks to provide a safe, clean, comfortable home for these staff members. Any issue not explicitly outlined in the agreement below will be addressed with the best interest of the facility, the farm and other residents in mind. Gorman Heritage Farm reserves the right to modify or alter any policy outlined below with written notice to applicants.
Upon arrival to the farm, residential employees will be given a door code for their living area. Please do not share this door code with anyone.
In the event of a key given, there is a $10 fee for any key which is lost or requires replacement. At such time as employment or volunteering with Gorman Heritage Farm ceases, the key(s) will be returned no later than the last day of employment or volunteer session. Any key(s) not returned will be subject to the replacement charge of $10 each. Keys must not be reproduced or copied at any point for any reason.
No pets or domestic animals (including but not limited to dogs, cats, birds, etc.) are strictly prohibited, both within residential and work areas as well as in the Gorman Heritage Farm Office.
Residents are expected to use discretion where noise is concerned both in and around on-site residential areas. Therefore, excessive noise is prohibited at all times. Courtesy hours are always in effect, and residents and guests are asked to be considerate of other's rights to sleep. Loud stereos, gathering in hallways, and other disturbances are discouraged. Due to the nature of these instruments, DRUMS AND AMPLIFIED GUITARS are not permitted in any on-site housing. Quiet Hours are to be observed as 10:00 p.m. to 8:00 a.m.
Noise, including talking, music, and other miscellaneous sounds must be kept at a minimum level so that they cannot be heard in other's rooms. If a resident has a problem with noise, there is an expectation that said resident speak with the resident or guest(s) who are creating the noise. If the noise continues after a resident has confronted the situation, it is expected that the issue be brought to the attention of the assigned staff member.
Gorman Heritage Farm assumes no liability for damage or loss of personal property. Residents are allowed to maintain one vehicle on site, but storage outside of each individual living space will be assessed on an individual basis.
Residents should not engage in activities that might place themselves or others at risk of bodily harm. This involves, but is not limited to, activities such as throwing things out of windows, sitting on window ledges, entering onto any roof areas, or the use of bicycles, rollerblades or skateboards within any residential area.
Gorman Heritage Farm is a 100% smoke-free site. This means that smoking is not permitted in buildings or on-site at any time. Evidence of smoking in a residential room is sufficient for a resident to be held accountable for violating this policy. Violators of the smoke-free guidelines also will be held accountable for their action. Common lounges, hallways, stairwells and laundry facilities are all non-smoking areas, and in front of the residential areas. Smoking must only occur in designated areas on site.
Pursuant to the Gorman Heritage Farm staff handbook and published policies and procedures, the possession of any dangerous instrument on-site is prohibited. These include, but are not limited to, any gun, rifle, pistol or other firearm (whether operable or inoperable, loaded or unloaded), dynamite cartridge, bomb, grenade, mine, incendiary or explosive device, BB gun, air rifle, air pistol, firecracker, knife, dirk, dagger, slingshot, leaded cane switchblade knife, blackjack, metallic knuckles, razor and razor blades (except solely for personal shaving) or any device which may readily be used as a weapon or to inflict or cause harm to any person or structure. Likewise, metal darts and dartboards are prohibited as well.
Guests and residents are responsible for the furniture in their room and residential living area; therefore, you should keep the furniture assigned to your room in your room. Using furniture from the floor lounges, laundry rooms, or other common areas for your room is not permitted without approval from a designated supervisor. In addition, Farm-owned furniture is not allowed outside of the residential areas. All areas of the farm, including common areas, must be left clean and in the same conditions as they were presented.
Candles, incense, oil lamps, etc. are not permitted in residential areas. These items produce an open flame or hotspot that produces smoke. They may inadvertently set off the smoke alarm system causing a false alarm. They may start a fire. You are encouraged to bring irons and curling irons that have an automatic turn off feature. No fireworks are allowed in or around any residential area. Motorized vehicles may not be parked at any location in or around the student residences other than authorized parking lots/spaces. Residents are not allowed to bring halogen lamps into the residence halls. Halogen lamps burn up to 10 times hotter than an ordinary light bulb, causing a potential fire hazard.
Residents are expected to observe all pertinent fire prevention policies, including but not limited to blocking fire exits or tampering with fire alarms.
Site and Residential Area Security
Residents are expected to observe and comply with all safety and security policies. Residents may not block open or tamper with locked doors or provide gate combinations or keys to unauthorized persons.
Residents are permitted to have visitors, but residents are 100% responsible for the conduct of their visitors. Visitors are not permitted at any point while summer camp or other children’s programs are in session. Residents are responsible for the conduct of their visitors. Residents may not have any overnight visitor during times when camp is not in session. Residents are expected to exercise good judgment and discretion in hosting visitors on-site. For day visitors, residents are asked to exercise restraint and consideration for other residents regarding the number of visitors and length of stay. Visitors are asked to wear farm appropriate clothing and not take photographs of program participants.
All adults are expected to dress in a way that is professional and farm-appropriate. Clothing should not have offensive language or imagery, should not show undergarments and should provide an appropriate amount of coverage. Close toed shoes are required in certain activity areas and shoes are required throughout the farm. Adults should role model sun safety, and dress appropriately for the weather (i.e. long sleeved shirts and pants at night, rain coats etc). Each staff and volunteers will be provided with a name badge that must be worn at all times on the farm and visitors will be given a visitor badge upon checking in at the farm office.
Residents are provided a wireless connection to the internet at all times. Abuse of network resources will not be tolerated, including any activities that are considered detrimental to the network or cause excessive traffic. Residents are responsible for any activities conducted through the use of the wireless including actions taken by guests or others having access to the computer of a resident. It is recommended that residents take suitable precautions by restricting access to their personal devices. No illegal activity should be pursued, nor should a user enter any computer system or access any files stored on a computer without the express permission of the owner of the files and/or computer.
Farm staff may enter the living space of a resident in the event of a serious life or health-threatening emergency at any time, including when the resident is not present. Authorized personnel as previously mentioned may enter the living space of a resident to perform requested, preventative, prescheduled or emergency maintenance, or, in the absence of the occupant, to silence a disruptive noise. When possible, advance notice of room entry will be given. Rooms may be searched by appropriate legal agencies with a warrant or with your permission. Room inspections will be limited to those times when you have given your permission, or when authorized staff have reasonable evidence to believe a search is necessary to resolve a serious life or health-threatening situation. Property may be relocated from the living space of a resident if it constitutes an imminent danger.
All trash is to be bagged up and moved to the Interpretive Center parking lot dumpster once a week.
Drug and Alcohol Policy
All residents are subject pursuant to Gorman Heritage Farm’s existing drug policy (outlined both in the staff manual and the employee handbook). Alcohol must be consumed in moderation, must be kept inside of the farmhouse at all times, must be kept in unoriginal containers when being consumed, and all rubbish disposed of.
Cleaning and Check-Out
Residents are expected to maintain a reasonably clean and healthy environment within their living space and common areas, including but not limited to regular waste disposal, dusting, vacuuming, etc. A vacuum is provided for residents as is a laundry facility. However, it is the expectation for residents to provide their own cleaning supplies for cleaning their living spaces.
Upon check-out, residents are expected to return their living space to the level of cleanliness demonstrated at check-in. Authorized Farm staff will assess the cleanliness of the room and any damage or maintenance related issues.
There are specific bicycles around the farm that are to be used for emergency procedures and should not be used for any other purpose. The rider must always wear a helmet.
Wildlife & Farmyard Animals
At all times, staff are expected to respect wildlife and farmyard animals. This includes demonstrating care for all animals, and not killing, trapping, or otherwise harassing farmyard animals/wildlife.
Golf Carts and Other Vehicles
Staff and volunteers who hold a full driver’s license and have been signed off by a Staff Member (VEHICLE COMPETENCY FORM) may drive golf carts or other smaller vehicles. Golf carts are needed for moving equipment to different activity areas. Seatbelts should be worn at all times, speed should never exceed 10 mph and keys should not be left in vehicles. Farm visitors are not allowed to play on the golf carts, even when not in use. Please check with a member of the Farm staff before using a golf cart, to ensure it is not allocated to another member of staff.
Staff and volunteers must leave vehicles in the farmyard parking lot or the Interpretive Center Parking Lot. No personal vehicles are allowed on the farm site.
Sign in/out Sheet
All staff and volunteers are expected to sign-in and sign out when leaving/returning to the farm property. The sign in/out sheet is located in the Farm House. This is the official record of who is on site and is important information in the event of an emergency.
All staff and volunteers are expected to conduct their personal relationships in a manner that does not interfere with professional responsibilities. Personal relationships should not be displayed publicly in front of program participants or other staff. Staff and volunteers should be conscious of their interactions with other staff, in front of program participants, and maintain an environment of professionalism at all times.
2 Adult Policy
With 1 or more children, adults MUST be within eyesight of another adult.
The Farm is a smoke free site; alcohol and illegal drugs are not permitted on-site at any time.
In the interest of the health and wellness of campers and staff, there are no animals allowed onsite at any time, unless as a prearranged part of the camp program. Any requests to bring a personal animal to camp must be approved by the Executive Director.
All staff are expected to dress in a way that is professional and farm appropriate. Clothing should not have offensive language or imagery, should not show undergarments and should provide an appropriate amount of coverage. Closed toed shoes are required and shoes are required throughout camp. Adults should role model sun safety, and dress appropriately for the weather (i.e. long sleeved shirts and pants at night, rain coats etc). Each staff and volunteer will be provided with a name badge that must be worn at all times during the session and visitors will be given a visitor badge upon checking in at the camp office. Staff and CIT volunteers will also be provided Camp T-shirts that they are required to wear on specific days each week. It is recommended that any dangling jewelry not be worn to ensure that it not catch on anything.
Staff, CITs, and volunteers are asked to arrive at camp at their designated time on days they are scheduled to work and to stay until their scheduled time. In the event staff, CITs, and volunteers are not able to do so, they are asked to contact the Camp Director immediately.

Communication with Campers
We appreciate that camper and staff develop a unique and trusting relationship during their time at camp. While these relationships are positive, we discourage them from continuing outside of camp. Campers and parents/guardians are made of aware of this in the Camper Contract. It is expected that staff will not share personal contact information with campers under 18 years old including phone numbers, email, postal addresses and “friending” campers on social networking sites.
Staff are entrusted with sensitive information about their campers, in order to provide the best possible care while at camp. This information should not be shared with others who are not immediately involved in the care of the camper. This information is never to be shared with other campers, parents, visitors or others not affiliated with camp. When discussing a specific camper with other staff, use discretion to assure information is kept confidential. In matter of child protection, a staff must never promise to keep secret any information that is divulged. All information regarding concern or assessment of child abuse should be shared on a “need to know” basis in the interest of the child; the giving of information to the Mandated Reporter, for the protection of the child, is not a breach of confidentiality.
Staff and participants are asked to show respect toward our farmyard animals at all times. Staff are also asked to talk quietly and calmly, and to move slowly and carefully. Always shut gates after you enter or leave an animal enclosure.
Gators and Other Farm Vehicles
Staff that hold a full driver’s license and have been signed off by a full-time staff member may drive Farm vehicles. These vehicles are needed for transporting campers with limited mobility, or in the event of an emergency, or for moving equipment to different activity areas.
Seatbelts should be worn at all times, speed should never exceed 10 mph and keys should not be left in vehicles. Campers are not allowed to play on any farm vehicle, even when not in use. Please check with the Camp Director before using a farm vehicle, to ensure it is not allocated to another member of staff at that time.
Staff will assume responsibility for good general hygiene for themselves and the campers. This includes hand washing after handling animals. It is the responsibility of all staff to make sure that their area of camp is kept clean.
Inclement Weather/Tornado Plan
In the event of inclement weather, staff are asked to wait 15 minutes after the last roar of thunder to move from their location. In the event of a tornado, staff members in the Interpretive Center will inform visitors and campers to take cover in the men and women’s restrooms inside the Interpretive Center. The men’s and women’s restrooms are located to the left of the main entrance. Get as far away from the door as possible, crouch down, and cover your head with your arms. Staff members will inform you when it is safe to leave the bathrooms.
In the event of a tornado while at the farmyard, staff members will inform visitors to take cover under the Farm Office. The farm office is located across from the Alfalfa barn. Get as far away from the opening, crouch down, and cover your head with your hands.
Vulgar or offensive language is not permitted. We believe in using person centered language at all times, with campers and staff, and maintaining a level of respect in all of our vocabulary around the Farm.
If living on-site, all medication must be kept locked safely in the farmhouse
Any personal property brought to the Farm is the responsibility of the owner. No items should be brought to camp that could inflict injury to another. Personal activity equipment should not be used with campers unless agreed upon ahead of time by the Camp Director.
Personal Vehicles: Staff must leave vehicles in the staff parking lot. No personal vehicles are allowed on-site, when in session, nor for transporting campers.
Cell phones are not to be used during the camp program but staff are asked to keep their phones on them in the event of an emergency; it is not appropriate to use a cell phone as a watch. Campers are told to not use cell phones so please be respectful of this policy.
Out of respect for our campers’ privacy, we ask that no videos be taken while at camp. Photos may be taken for personal use only; photos and names of campers must not be put on the internet. We encourage staff to be aware of the privacy of their fellow staff, and to share photos with Gorman Heritage Farm to share. We ask that no pictures or videos be shared online at any time.
Staff are expected to be on time for work and for all activities.
We encourage staff to be aware of their privacy settings as regards social networking sites, personal websites etc. Having public pages mean that campers and parents can access personal information. When referencing Gorman Heritage Farm, keep in mind conduct that can then be associated with camp (such as obscenities, profanities, derogatory comments and other such conduct that would not be tolerated at camp).
We encourage staff to bear in mind that once identified as a staff member at GHF, you are seen as an ambassador of camp to the campers and staff that you have met here. If staff identify themselves with Gorman Heritage Farm on social networking sites, blogs etc. it is expected that they will not engage in behavior (comments, photos) that is prohibited by camp policy (including but not limited to sexually explicit, suggestive, humiliating, demeaning, derogatory, profane or vulgar behavior).
All staff are expected to conduct their personal relationships in a manner that does not interfere with professional responsibilities. Personal relationships should not be displayed publicly in front of campers or other staff. Staff should be conscious of their interactions with other staff, in front of campers, and maintain an environment of professionalism at all times.
Permission of the owner is required for anyone taking or possessing the personal property of another.
For staff living on-site, visitors to staff on-site must be approved by the Camp Director and must be accompanied by a staff member at all times. Visitors will check in at the camp office, sign-in and wear a visitor badge while onsite. Visitors are asked to wear camp appropriate clothing and not take photographs of campers.
Gorman Heritage Farm strives to provide and maintain a safe, secure, caring environment in which children can learn and thrive. Gorman Heritage Farm will take all steps possible to ensure the safety of all individuals who participate in its programs, including: minimizing and/or eliminating safety hazards, providing close supervision of children, setting and enforcing appropriate limits on behavior, and providing guidelines on how to handle emergency situations.
Gorman Heritage Farm seeks to prevent child abuse in any form to individuals who participate in Gorman Heritage Farm programs. Staff and volunteers are expected to appropriately meet the needs of children and youth. In addition, as a provider of child care and educational programs for all ages, Gorman Heritage Farm is subject to certain legal obligations under Ohio law with respect to reporting of suspected abuse or neglect of children.
Under Ohio law, an abused child is defined as one who is the victim of sexual activity, is endangered or neglected, or suffers physical injury (except as to proper corporal punishment by a parent or guardian), mental injury, or death other than by accident. Child abuse may be physical, sexual, emotional, or mental, and may be the result of actions (i.e. hitting, sexual contact, or verbal attacks) or omissions to act (i.e. failure to care for a child or to provide for the child’s well-being).
Ohio law imposes a mandatory requirement to report suspected abuse or neglect of children for certain persons who work with children. Section 2151.421 of the Ohio Revised Code states that:
“No attorney, physician, … administrator or employee of a child day care center, administrator or employee of a certified child care agency or other public or private children services agency, school teacher, school employee, school authority, … who is acting in his official or professional capacity and knows or suspects that a child… has suffered or faces the threat of suffering any physical or mental wound, injury, disability, or condition of a nature that reasonability indicates abuse of neglect of the child, shall fail to immediately report that knowledge or suspicion to the appropriate authorities.”
Known or reasonably suspected child abuse will be reported immediately to the Gorman Heritage Farm Executive Director, Program Manager, or designated child advocate. Objective information concerning the allegation will be documented in a written incident report. Only those with a reasonable need to know will be notified of a report.
A report must also be made in person, by telephone, or in writing to the children’s services agency in the county where the child resides or was allegedly abused or to a local law enforcement agency (such as the Hamilton County Sheriff). In Hamilton County, reports can be made by calling (513) 241-KIDS, 24 hours a day. The report should contain the following information: name and address of the child, age of the child, name and address of the parent or guardian, name and address of the alleged abuser, reason you suspect the child is being abused or neglected, the nature and the extent of the injury or abuse, as well as any other helpful information you may have. You may report anonymously, but you are encouraged to give your name. Any report is confidential and the person making the report is immune from civil or criminal liability for making such a report.
Prevention of Child Abuse at Gorman Heritage FarmIn an effort to create the safest possible environment within Gorman Heritage Farm, several abuse prevention measures will be utilized. These measures include screening of all staff members who are responsible for child or youth programs, screening of volunteer educators and leaders, provision of regular training on child abuse issues for paid staff and volunteers, standards for appropriate discipline methods, and program guidelines which support good supervision and safe interactions.
ScreeningAll paid staff members are required to complete a written application and provide references (which will be checked). A criminal background check or police check may be conducted; therefore the employee (or potential employee) may be required to sign a Personal Information Release Form.
Prospective volunteers are required to complete a written application. Those applying to volunteer with children or youth will be required to sign a self-disclosure statement indicating whether he or she has ever been convicted of child abuse or a similar offense, or had such a conviction expunged from their record. Anyone who has had a child abuse conviction or expungement, as well as anyone refusing to sign the statement, may be restricted from working with children. At any time, a volunteer might be required to provide a set of impressions of his/her fingerprints and a criminal record check might be conducted with respect to the volunteer. In addition, all volunteer educators or group leaders of children or youth will be required to read Gorman Heritage Farm’s youth protection policy for child safety and child abuse prevention, and sign a form indicating that they have read and understand the policy and agree to abide by it.
TrainingPaid and volunteer staff who work closely with children or youth should attend a GHF training session related to child safety and the child abuse prevention policy. Additional training on risk management, emergency preparedness, first aid, CPR, and group management are also recommended.
Types of Prohibited Behavior
Physical abuse: Any non-accidental or deliberate injury to a child by someone responsible for the child’s care. Physical abuse includes hitting, kicking, slapping, punching, or similar acts which cause or could cause serious physical injury to a child. No corporal punishment is allowed at Gorman Heritage Farm.
Emotional abuse: Any behavior, including verbal assaults or excessive demands on a child, which negatively impacts the child’s mental health or social development. Any behavior on the part of the group leader which attacks a child’s self-image (belittling, ridiculing, name-calling, shaming, or sarcasm) is prohibited.
Neglect: Not meeting the child’s basic physical and emotional needs, as far as is possible in the short period of time the child is at Gorman Heritage Farm. Be aware of the child’s needs as related to food, water, energy level, physical discomfort due to weather, and medical concerns. Staff should never leave children or youth without appropriate supervision.
Sexual abuse: Any sexual act between an adult or older child and a child. Sexual contact includes fondling or any other inappropriate touching of private body parts. Sexual abuse also includes exhibitionism, voyeurism, and any form of sexual exploitation of children. Follow guidelines for safe, appropriate touching. Do not spend extended periods of time with a child or group of children without another adult or leader present.
What Do You Do When A Camper Discloses or You Suspect Abuse?If you suspect a child may have been abused, or a child discloses to you that they have been abused (at home or while at camp), we have a legal obligation to report it. Please follow these steps:
Listen—Listen to what is being said without displaying shock or disbelief. Please don't ask any questions. We ask that you don't solicit any information. Never promise at any time, that you can keep a secret. If a camper asks, state: “I can’t promise that I’ll keep a secret because I may have to tell someone so that we can help you. I do want to hear what you have to say.” Report it to the Camp Director immediately.
Reassure—Reassure child, but only so far as is honest and reliable (don't make promises you may not be able to keep like "Everything will be okay now"). Say, "I am glad you told me. I believe you."Record—Make some notes right away, recording actual words used, statements made, observable and nonverbal behavior.
The Camp Director will report the incident to the authorities if appropriate. Under no circumstance should anyone report an incident without notification and permission of the Camp Director and Medical Director.
Do not discuss with other staff members. Confidentiality is crucial.
Allow the campers to put on their own sunscreen whenever possible. Always have other campers/staff present when applying sunscreen to campers.Child Abuse is a Serious Criminal Offense
As a staff with the responsibility of caring for children, you may be placed in a sensitive situation, making you vulnerable to charges of child molestation. Sexual exploitation should not be confused with physical contacts that are true expressions of affection.
A warm and healthy relationship can exist between the camper and camp staff if staff members respect the child and place reasonable limits on their physical interaction.
