Shortly after you receive this form we'll assign a discount code that is unique to you. From that moment on, when your students use that code they will receive a 15% discount on any purchase they make on our website at any point during the school year. In addition, there are 3 "Book Special" months when we increase the discount to 20% and up to 35%. By default, the "Book Special" months are OCTOBER, DECEMBER and APRIL. If that schedule works for you, please leave this blank. If you prefer to have the "Book Special" on different months, feel free to select them below.

For the "Book Special" months, we offer 2 delivery options: (1)we ship all the books that your students ordered to your classroom so you can distribute them, or (2)we ship the book orders directly to each of your students' addresses. Please select your preference.
