Media Release Waiver

I hereby authorize any images or video footage taken of my youth (under 18 years of age), in whole or in part, individually or in conjunction with other images and video footage, to be displayed on any social media platform, including Basketball Manitoba's websites and other official channels, and to be used for media purposes including promotional presentations and marketing campaigns. I also authorize the display and use of any media material created by my youth within the Winter Elite Basketball Camp.

I waive rights to privacy and compensation, which I may have in connection with such use of my youth’s name and likeness, including rights to be written copy that may be created in connection with video production, editing and promotion therewith.

I am over 19 years-of-age and the parent or legal guardian of the youth, and I have read this waiver and am familiar with its content.
Liability Waiver

Winter Elite Basketball Camp, it's affiliates and any facilities where camps will be held will not be responsible, and will assume no liability for injury or damages arising from the results of the above named Athlete’s participation unless due to the willful misconduct or gross negligence on the part of Winter Elite Basketball Camp, its affiliates, or agents. Due to the strenuous nature of basketball, the Athlete participating and their parents are urged to consult their physician concerning the Athlete’s fitness to participate. Basketball presents certain inherent risks and hazards, which the participating Athlete is urged to consider and which the Athlete assumes the outcomes of such risks and hazards. I hereby approve of the participation of my child, the above named Athlete, in the Winter Elite Basketball Camp and consent to the emergency medical treatment for my child on my behalf, if it is needed. To the best of my knowledge, there are no physical or other conditions which will interfere with my child’s participation.
