These questions are aimed at making sure you realise the extra responsibilities that come with owning a dog, to help you determine what type of dog you are looking for, and to assist us in finding the right dog for you. For example, if the dog you are seeking is to be a 24 hour inside dog then an outside active dog would not be appropriate. Please ensure you include all relevant information in this application. Only that information which is provided in this application will be considered in assessing suitability of your chosen dog(s).

Please understand that popular dogs may receive up to 100 applications and we only telephone at the most three applicants. We must make our decision on whom to contact from the information you give us. If you are not one of those called, this does not mean you are not a good applicant, but based on the information you gave us you do not seem the best match for this particular dog. If you are not naturally a wordy person, we appreciate it can be difficult, but please try. This application can then be used for any other of our dogs or cats on the same basis.

Does your home have a back yard and is it private or shared?
If your home is an apartment, what is the proposed toileting arrangement for your new dog?
Is your back yard separated from your front yard? Does it have a lockable side gate?
Is your yard securely fenced? Describe type of fencing (wooden, colourbond, wire etc) and height at the lowest point. Note:If your fencing is not secure, please do not apply until it is
Do you have a swimming pool, and is it separately fenced?
Does your house have any stairs? Internal or external? If so how many? Can you please describe them (type of stairs and number of stairs)?
If no-one is at home, where will the dog stay? Inside or Outside? If the dog is outside, what type of shelter will the dog have?
How many hours will the dog be without human company for each day?
How often will you have time to walk the dog? How long for? On lead or off lead?
Under what circumstance would you give up this pet, e.g. family illness, barking, digging, shedding, allergic to new dog, destructive chewing, biting, growling, moving house, reactivity on lead, lack of house-training, high cost vet bills?

If your adoption is successful do you agree to leaving VicDRG as the emergency contact on the microchip registry? (This is in case your pet goes missing and you are uncontactable, we will always be able to assist).
Do you currently have a vet? If so, please provide us their name and phone number so we can contact them for a reference, together with one personal referee. If you do not have a vet please provide two personal references. We reserve the right to contact these referees should the application proceed.
Do you have any medical conditions that might mean you’ll require assistance at times with your adopted pet, for example, in dog walking, attending vet visits, etc?(Please understand your answer is confidential and this question is simply aimed at knowing how we can support you best as a valued adopter.)
Is there anything else you would like to tell us about you, your family or your interest in adopting a dog? The more information you provide, the better we will be able to determine suitability of your chosen dog(s).

By submitting this application, I agree that all the information I have provided is correct. Providing untruthful answers or failure to comply with the requirements of this application or the signed adoption contract can result in the forfeiture of the adoption.

You agree to allow us to check the references provided. We will require the successful applicant to sign an adoption agreement and allow a property check before adoption.

We may also ask to sight your driver’s licence before the application proceeds.

We appreciate you taking the time to complete this application. Upon review and approval, we will contact you as soon as possible.

We appreciate that it is a long application to complete if the application is unsuccessful. We hope by completing it you have found out more about your requirements for a dog.

If you reapply for a further dog from us there is no need to complete another application form. Please advise us that we already have your form.

We reserve the right to refuse any applicant and will not enter into any further correspondence on our decision.
