Volunteer Waiver for Festival Park, LLC

In consideration for the opportunity to volunteer at the Racine Civic Centre, I agree to the following:

Release and Waiver of Liability:

I forever release and waive any rights, claims, and causes of action for damages that I, my heirs, executors, administrators, and assigns may have, or that may hereafter accrue against the following (collectively, the "Released Parties"): Persons, organizations, charities, and other entities associated with the event, including supporting sponsors and their respective employees and agents; individuals in any way connected with this event or future events (including volunteers); 5Kevents.org; Festival Park, LLC; the City of Racine; sponsors; affiliates; volunteers; and individual organizers.

This release applies to any damage arising from or in connection with my volunteer involvement before, during, or after the event.

Assumption of Risk:

I certify that I am physically fit to participate in this volunteer activity and assume all associated risks.

Media Release:

I grant permission to free use my name and/or likeness in telecasts, broadcasts, newspapers, posters, advertising, and other media.

Limitation of Liability:

5Kevents.org shall not be liable to me for any direct, indirect, special, incidental, consequential, or exemplary damages, including, but not limited to, loss of profits, goodwill, use, data, or other intangible losses.

Governing Law and Dispute Resolution:

This waiver will be governed by and construed under the laws of the State of Wisconsin, USA. If a dispute arises out of or relates to this waiver or its breach, and if it cannot be settled through negotiation, the parties agree to first attempt to resolve the dispute through mediation before resorting to arbitration, litigation, or other dispute resolution procedures.
