REQUIREMENTS:
- Participant applicants must be actively enrolled in higher education.

Free event for International Students currently or previously enrolled in TWTS. Other International Students fee is $20.00 payable during registration. OtherDomestic students not registered in TWTS cost is $40.00.

NOTICE:

For your convenience, you will be able to pay your Retreat fee, if required, at the completion of this form. Credit and Debit cards will be accepted.

GENERAL LIABILITY RELEASE:

a. I release CEO Global, CEO Global USA, Sunshine International Camps, CJ Camps, Real Leadership Retreats and its staff, affiliates and volunteers from any and all liability regarding any accident, incident or illness that may occur during the duration of the program.

b. I understand that participating in a large student program and the activities involved are inherently risky and I agree to take full responsibility for my health, safety and well being during the program. I also understand that CEO Global, CEO Global USA does not provide medical insurance and that I am responsible for all of my own medical expenses that may be incurred.

MEMORANDUM OF UNDERSTANDING:

By submitting this application, I attest that the information represented here is true and accurate to the best of my my knowledge. I commit to comply with the following rules and any other official request from the staff and directors of the program.

I UNDERSTAND:
a. That applying for this this program constitutes a real and serious intention to participate in the program that for which you are applying. CEO Global takes this commitment and intention seriously and organizes programs and resources based on applicant commitments.

b. I understand that by enrolling in CEO Global USA programs I am agreeing to allow my image, likeness and quotes to be used in public communication such as but not limited to; video, audio, photos, publications and websites.

I AGREE TO:
a. Complete all pre-program training provided by CEO Global (staff & mentors only)
b. Maintain a positive attitude and a willingness to serve other participants, staff and mentors at all times.
c. Follow instructions from staff & mentors with a good attitude.
d. Maintain a positive attitude about education and learning as well as respect the learning environment at the camp.
e. Refrain from entering into romantic relationships with any other staff person, mentor, or student while at the camp program.
f. Abstain from all alcohol or smoking while at the camp programs.
g. Abstain from any immoral behavior, or the appearance thereof.
h. Recognize that engaging in any immoral behavior will result in being sent home early at your own expense.

NOTE: CEO Global reserves the right to Expel program participants for reasons including but not limited to: continued failure to follow instructions, engaging in unethical behavior, immoral or inappropriate behavior towards others, disrespecting staff or for negative or rebellious attitudes.
I agree with the Memorandum of Understanding
FINANCIAL AGREEMENT:

a. I understand that any and all application fees and camp fee payments made towards involvement with CEO Global programs are non refundable unless the program is completely cancelled.

b. I understand that in the event of cancellation, postponement or relocation of a program I must adjust my travel arrangements at my own expense.

c. I understand that CEO Global does not provide medical insurance coverage and that I am responsible for arranging travel medical insurance and taking responsibility for any and all medical expenses related to my time in the program.
