Applicant has read the attached Policy and Guidelines for Conducting Special Events, Benefits or Promotions to Benefit the Phoenix Society for Burn Survivors and agrees to abide by them. Applicant understands that approval must be granted by the Phoenix Society and a Letter of Agreement must be executed by the parties before Applicant can plan or promote the proposed event.
A final accounting report from the event organizer is required within 30 days of the event. Funds are to be given to the Phoenix Society no later than 30 days following the event. Phoenix Society will not cover any losses from the event and is not liable to any party or vendor for fees, costs, or payments of any kind. The organization and/or individual sponsoring the event agrees to indemnify the Phoenix Society for Burn Survivors and any officer, director, employee, member, agent or volunteer from: (i) any and all charges, costs, claims, liabilities, damages, costs, obligations, debts, and expenses arising out of or in connection with the event and (ii) any judgments, penalties (including excise and similar taxes), fines, settlements, and reasonable expenses (including attorney fees) incurred in connection with any threatened, pending or completed action, suit or proceeding arising out of or connected with the event, whether civil criminal, administrative, arbitrative or investigative.
