IMPORTANT: Your REUNION FEE ($75 per person) includes the snacks and drinks at the Hospitality Suite (Friday and Saturday) and the Banquet Dinner on Saturday night. The total fee for you and your spouse or guest (if any) must be received by the Association Treasurer, Jerry Darr, not later than 11 Aug 2018. NOTE: The deadline for submitting this form is 27 July 2018.
NOTE: This Reunion Registration Form is NOT a room reservation at the venue. Each attendee must make his/her own room reservations. If you are staying at the venue, ensure you get the Association's special room rate.
Make your bank check (or bank bill-pay) payable to "AZ ASA Assoc." Note in the check's memo line that the payment is for "2018 Reunion." Mail your check to:
