Welcome to the new semester at UC Blue Ash!
After having registered with our office to receive accommodations, please use this form to request your accommodations every semester thereafter.
The four-step process:
1. Student completes and submits the New Semester Accommodations Letter Request.
2. Student receives, signs, and forwards the accommodations PDF to their instructor.
3. Student prints a copy of the form and has instructor to sign and return it to Accessibility Resources.
4. Both student and instructor retain a signed electronic copy for their records.

We will send your updated Accommodations Letters to your UC email within 72 hours.
