Annual Physical Form

The Commonwealth of Massachusetts requires that we have on file a current, no more than 12 months old, annual physical form and complete immunization records for your child. Your child will not be able to start school until we have this on file and will not be able to return to school should we not receive a new one when it expires. If you do not have a copy, your child's doctor can fax on to the office, our fax number is 508-590-6644.
