SERVICE AGREEMENT - Life Changing Services, Eternal Warriors
We welcome you to Life Changing Services, and hope that your time with us will be worthwhile. The following information is important for your consideration;
Mentor Vs Therapy
You will be taught by a trained mentor. These mentors are not a psychotherapist or physician, and are not trained in diagnosing psychological or medical conditions. If any issues arise for you that we believe would be best handled by a licensed therapist or physician, we will recommend that you contact the appropriate professional.
Goals
You will be asked to set goals during your class. You will determine the nature and amount of change you wish to make. Your mentor may make suggestions, but ultimately it will be up to you to set and follow through with your goals.
Grievance
You have every right to be treated with respect and dignity in a safe environment. Discrimination by our staff is not tolerated, if you have concerns about the services you receive, please contact our office.
Registration and Mentoring Fees
Once you have paid your initial registration fee to the Eternal Warriors program, your mentor will collect your program Mentoring Fee. You can choose to pay the upfront fee or you can choose to split your fee into three monthly payments as shown.
If you are enrolled in group coaching, once you begin group sessions you will be counted as present for each week that you are enrolled in the program whether or not you are in the live session, and you will receive a recording of the session to watch.
If you are enrolling in a one-on-one or family option, you will meet at a time and frequency that you and the mentor are available. You must cancel any scheduled sessions with your mentor within 24 hours of the scheduled session.
If you need to defer your program for any reason, contact your mentor to make an arrangement and to discuss possible deferrment of monthly payments.
Generally, we do not issue refunds for a partially completed program. Please make every effort to complete the program once you have registered. Special circumstances may apply. Please contact your mentor if you have a special need to withdraw.
Our Commitment
We cannot guarantee results. However, we have seen numerous clients create powerful results by having the courage and determination to apply the principles we discuss. Our mentors are trained to provide a system for achieving greater self-mastery. Your mentor will bring attentive listening, understanding, and belief in you when you may have a hard time believing in yourself. You can expect your mentor to offer fresh perspectives, make requests, suggest action steps, acknowledge your wins, and guarantee utmost confidentiality (to the fullest extent of the law, and so long as we don’t fear for your or another’s safety). Our experience has shown over and over again that this works if you work it.

Student Commitment
I am motivated and committed to taking action on my goals. I realize that anything less than my intentional full participation will not lead to my success. I accept full responsibility for myself and any actions I take that might result from coaching.
I agree to make payments on the agreed upon dates set by the Eternal Warriors program and by my mentor. I agree to provide an updated credit card in the event my card on file is rejected for any reason. I agree to honor my scheduled session times.
In order to maximize the results I want to create, I understand that this is a no quitting contract and that I will not be refunded for sessions that I've completed with my mentor. I intend to engage fully in this coaching process for the duration of my contract.
