We {parentsNames} (“Parent” includes all parents or authorized adults listed here) have enrolled our child/children {campersNames} (“Camper” includes one or more children listed here) in St. John’s Parish Day School (“SJPDS”) summer camp (“Camp”). Parents authorize their Camper to participate in all activities during camp hours. If enrolled in full day Camp, these activities may include JumpBunch, cooking class, water play and Bricks4kidz. By signing this release and consent form, we acknowledge and accept the policies set forth below.
Fees and Forms. No Camper will be allowed to attend Camp unless all Camp fees have been paid and this form, the Camper Health History Form, and Medication Administration Form (only for Campers requiring medication during camp hours) have been duly signed and submitted.
Sign In/Out. Each Camper shall be signed in and out each day by a Parent or other authorized adult. A photo ID may be requested.
Medical Care. Camp employees will take all reasonable care to prevent accident and injury. However, accidents and injuries can happen. In the event of a medical situation, Parent gives permission to have medical assistance given to Camper. If Parent cannot be reached and emergency care is needed, Parent gives permission for Camper to be taken to the nearest emergency room. The hospital has Parent’s authority to provide all medical care they deem necessary for Camper. Parent assumes full responsible for all medical expenses that may result from such a situation.
Allergens. Camp strives to provide a nut-free environment for Campers who may be allergic to peanuts or nuts. Parent agrees that Camper will not bring peanut or nut-based products to Camp.
Safe Behavior. SJPDS reserves the right to dismiss any Camper whose behavior interferes with their own or other Campers safe or peaceful enjoyment of Camp. No refund of fees will be given for such a dismissal.
Sunscreen. Parents should apply sunscreen on Camper prior to Camp each day. If reapplication during camp hours is requested, Camp must have specific permission pursuant to a general memorandum issued by the Maryland Department of Health and Mental Hygiene on July 2, 2011.
