A nomination fee of $50.00 must be included with this form and mailed with a postmark no later than June 5, 2020.  Nominations postmarked after this date will be subject to a $200.00 late penalty and must be in by June 19, 2020 or they will not be accepted.  Please make the nomination fee payable to the American Road Horse and Pony Association (ARHPA).  The horse owner, trainer, and exhibitor must be a current member of both the USTA and the American Road Horse and Pony Association.  Papers must be submitted with the nomination form (in the current owner’s name—no exceptions).  All forms must be completed and have all copies of necessary documents. If not the nomination will be considered incomplete and will be subject to no acceptance.
