Terms and Conditions: This Facility Use Agreement is between Blue Mountain Community College and the organization/individual identified on this form, hereinafter referred to as “User”. The User agrees to be responsible for the proper care and use of the facility and the equipment. The User assumes the responsibility for damage to the facility and cost of repairs beyond normal use. The User agrees to use only the facility, equipment, and/or space approved for, per this agreement, to utilize for only the purpose and the location as described above.
The User is subject to College policies and procedures while utilizing designated facilities, space, and equipment. The College reserves the right to charge a surcharge for equipment or facilities used past the dates indicated above. The User must show proof of liability insurance for the requested activity and must comply with the approved usage outlined in this Agreement.
Be it further understood that Blue Mountain Community College assumes absolutely no financial responsibility for any injury, accident, or lawsuit that may occur during the User’s occupancy and/or usage of equipment, facilities, and space.
I understand and agree to the terms and conditions stated above and agree to pay the fees as listed below prior to gaining access to the approved College equipment, facilities, or space.
